Indications/Uses
Dosage/Direction for Use
1 tab daily at the same time w/o interruption for 28 consecutive days. No hormonal contraceptive used in the past mth Start on the 1st day of menstrual bleeding. When changing from a combined hormonal contraceptive (combined OC, vag ring or transdermal patch) Start on the day after the last active tab of combined OC, or on the day of removal of vag ring or patch. When changing from progestogen-only method (minipill, inj, implant) or progestogen-releasing IUS May switch to Exluton any day. Following 1st-trimester abortion Start immediately. Following delivery or 2nd-trimester abortion Start at day 21-28 after delivery or 2nd trimester abortion. Missed doses: <3 hr late: Take as soon as it is remembered. >3 hr late: Take as soon as it is remembered, additionally use a barrier method for the next 7 days.
Administration
May be taken with or without food: Take at about the same time each day. Swallow whole w/ water.
Contraindications
Hypersensitivity. Active venous thromboembolic disorder, known or suspected sex-steroid sensitive malignancies, undiagnosed vag bleeding. Presence or history of severe hepatic disease. Known or suspected pregnancy.
Special Precautions
Breast cancer, liver cancer, chronic disturbances of liver function, thrombosis, ectopic pregnancy, epilepsy, TB, high BP, chloasma (avoid too much exposure to the sun or UV radiation), diabetes. Consider discontinuation if sustained HTN or significant increase in BP not responding to antihypertensive therapy occurs. Risk of breast cancer. Rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption. Lactation.
Adverse Reactions
Increased/decreased wt; headache, migraine; contact lens intolerance; nausea, vomiting, abdominal pain, diarrhoea; rash, urticaria, erythema nodosum, erythema multiforme, chloasma; fluid retention; hypersensitivity; vag discharge, amenorrhea, irregular menstruation, breast tenderness, enlargement & pain; depressed mood, mood altered, increased/decreased libido.
Drug Interactions
Increased clearance of sex hormones w/ hydantoins, barbiturates, primidone, carbamazepine, rifampicin & possibly also oxcarbazepine, topiramate, felbamate, ritonavir, nelfinavir, griseofulvin & St. John's wort. Reduced absorption of steroid & reduced contraceptive efficacy w/ medical charcoal. May affect plasma & tissue conc w/ cyclosporin.
