Indications/Uses
Prophylaxis of venous thromboembolic disease associated w/ orthopedic or general surgery, medical patients bedridden due to acute illness, including cardiac insufficiency, resp failure, severe infections, rheumatic diseases.
Dosage/Direction for Use
SC Prophylaxis of venous thromboembolic disease in surgical patients 0.2-0.4 mL once daily. For general surgery, 1st inj should be given 2 hrs before surgical procedure. Orthopedic surgery 0.4 mL once daily initiated 12 hr preop. Prophylaxis of venous thromboembolic disease in medical patients 0.4 mL once daily for 6 days & continued for max of 14 days until the return of full ambulation. DVT w/ or w/o pulmonary embolism 150 anti-Xa IU/kg daily or 100 anti-Xa IU/kg bid. Complicated thromboembolic disorders 100 anti-Xa IU/kg bid. Unstable angina & non-Q-wave MI 100 anti-Xa IU/kg every 12 hr + oral aspirin 100-325 mg once daily for 2-8 days. Prevention of thrombus formation in extra corporeal circulation during hemodialysis 100 anti-Xa IU/kg. Patients w/ high risk of hemorrhage 50 anti-Xa IU/kg for double vascular access or 75 anti-Xa IU/kg for single vascular access. Severe renal impairment 2000 anti-Xa IU once daily (prophylactic), 100 anti-Xa IU/kg once daily (therapeutic).
Contraindications
Hypersensitivity. Active major bleeding & conditions w/ high risk of uncontrolled hemorrhage including recent hemorrhagic stroke.
Special Precautions
Do not administer IM. Patients w/ history of heparin-induced thrombocytopenia w/ or w/o thrombosis. Intraspinal hematomas. Not recommended in prosthetic heart valve. Higher risk of bleeding in low-wt women (<45 kg) & men (<57 kg). Renal impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Haemorrhagic bleeding. thrombocytopenia. Pain, haematoma & mild local irritation. Bullous eruptions (rare) or anaphylactic reactions.
Drug Interactions
Systemic salicylates, ASA, NSAIDs including ketorolac, dextran & clopidogrel, systemic glucocorticoids, thrombolytics & anticoagulants.
