Indications/Uses
Major depressive episodes, panic disorder w/ or w/o agoraphobia, social & generalised anxiety disorder & obsessive-compulsive disorder.
Dosage/Direction for Use
Major depressive episodes 10 mg once daily for 2-4 wk. Max: 20 mg daily. Panic disorder w/ or w/o agoraphobia Initially 5 mg for the 1st wk before increasing to 10 mg daily. Max: 20 mg daily. Social anxiety disorder 10 mg once daily for 2-4 wk, may subsequently be decreased to 5 mg or increased to a max of 20 mg daily. Generalized anxiety disorder & obsessive-compulsive disorder Initially 10 mg once daily, may be increased to 20 mg daily. Elderly >65 yr Initially 5 mg once daily, may be increased to 10 mg daily. Reduced hepatic function & poor metabolisers of CYP2C19 Initially 5 mg daily for the 1st 2 wk, may be increased up to 10 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Nonselective, irreversible/reversible MAOIs or reversible MAO-A inhibitors (eg, moclobemide), drugs that prolong QT interval. Patients w/ known QT-interval prolongation or congenital long QT syndrome.
Special Precautions
Paradoxical anxiety. Avoid in patients w/ unstable epilepsy. Patients w/ history of mania/hypomania, diabetes, suicide/suicidal thoughts or clinical worsening, akathisia/psychomotor restlessness, hyponatremia, hemorrhage, electroconvulsive therapy, serotonin syndrome, St. John's wort. Discontinuation symptoms seen when stopping treatment, CHD, QT interval prolongation & angle-closure glaucoma. Discontinue if seizures develop for the 1st tie or increase in seizure frequency; in patients entering a manic phase. Pregnancy & lactation. Elderly. Childn & adolescent <18 yr.
Adverse Reactions
Headache; nausea. Decreased/increased appetite, increased wt; anxiety, restlessness, abnormal dreams, decreased libido in male & female, anorgasmia in female; insomnia, somnolence, dizziness, paresthesia, tremor; sinusitis, yawning; diarrhea, constipation, vomiting, dry mouth; increased sweating; arthralgia, myalgia; ejaculation disorder in male, impotence; fatigue, pyrexia.
Drug Interactions
Serious reactions w/ irreversible, nonselective MAOIs. Risk of serotonin syndrome w/ reversible, selective MAO-A inhibitor (eg, moclobemide); reversible, nonselective MAOI (eg, linezolid); irreversible, selective MAO-B inhibitor (eg, selegiline). Class IA & III antiarrhythmics, antipsychotics (eg, phenothiazine derivatives, pimozide, haloperidol), TCAs, antimicrobial agents (eg, sparfloxacin, moxifloxacin, erythromycin IV, pentamidine, halofantrine), certain antihistamines (eg, astemizole, mizolastine). May lead to serotonin syndrome w/ serotonergic drugs eg, tramadol, sumatriptan & other triptans. Medicinal products capable of lowering seizure threshold eg, antidepressants (tricyclics, SSRIs), neuroleptics (phenothiazines, thioxanthenes, butyrophenones), mefloquine, bupropion & tramadol. Enhanced effects w/ lithium or tryptophan. Increased incidence of adverse events w/ St. John's wort-containing prep. Altered anticoagulant effects. Increased bleeding tendency w/ NSAIDs. Alcohol. Concomitant use w/ hypokalaemia/hypomagnesaemia inducing medicinal products.
