Anidulafungin
Indications/Uses
Invasive candidiasis, including candidemia in adults & ped patients ≥1 mth. Esophageal candidiasis in adults.
Dosage/Direction for Use
IV Adult Invasive candidiasis, including candidemia 200 mg as a single loading dose on day 1, followed by 100 mg daily thereafter. Continue for at least 14 days after the last +ve culture. Esophageal candidiasis 100 mg as a single loading dose on day 1, followed by 50 mg daily thereafter. Duration of treatment: Min of 14 days & for at least 7 days following resolution of symptoms. Ped ≥1 mth Invasive candidiasis, including candidemia 3 mg/kg (not to exceed 200 mg) loading dose on day 1, followed by 1.5 mg/kg (not to exceed 100 mg) daily thereafter. Continue for at least 14 days after the last -ve culture & improvement of clinical signs & symptoms. Max rate of infusion: 1.1 mg/min. May switch to oral antifungal after a min of 10 days on IV therapy.
Contraindications
Hypersensitivity to anidulafungin or other echinocandin class (eg, caspofungin).
Special Precautions
Discontinue if anaphylactic reactions including shock occur. Infusion-related reactions. Not to be given in patients w/ hereditary fructose intolerance. Monitor for evidence of worsening hepatic function & evaluate for risk/benefit of continuing therapy in patients who develop abnormal LFTs during therapy. Pregnancy & lactation. Infant & childn <2 yr. Not recommended in neonates <1 mth.
Adverse Reactions
Clostridium colitis, fungemia, candidiasis, oral candidiasis; thrombocytopenia, thrombocythemia, coagulopathy; anaphylactic shock/reaction; hypokalemia, hyperkalemia, hyperglycemia, hypomagnesemia, hypercalcemia, hypernatremia; convulsion, headache; blurred vision, visual disturbance, eye pain; atrial fibrillation, ventricular extrasystoles, sinus arrhythmia, right bundle branch block; thrombosis, HTN, flushing, hot flush; bronchospasm; fecal incontinence, diarrhea, nausea, vomiting, constipation, upper abdominal pain; cholestasis; urticaria, generalized pruritus, rash, pruritus; back pain; infusion site pain; prolonged ECG QT, abnormal ECG & LFT; decreased blood K & Mg; increased blood creatinine, urea, amylase, alkaline phosphatase, & bilirubin; increased lipase, ALT, AST, γ-glutamyl transferase, hepatic enzyme, & transaminases; decreased/increased platelet count.
