Epinephrine
Indications/Uses
Initial treatment of all anaphylactic reactions except to epinephrine. Cardiac stimulant in case of collapse, shock & anesth accidents. Hemostatic in hemorrhages & prolongs action of infiltration anesth. Frequently employed in urticaria.
Dosage/Direction for Use
IM/SC/IV Anaphylactic shock 500 mcg IM inj repeated as necessary every 5 min. Emergency self administration: 300 mcg. Childn 10 mcg/kg by IM inj, depending on age & wt. Advanced cardiac life support Adult Initially 1 mg IV, may be repeated as often as every 2-3 min during resuscitation. Childn 10 mcg/kg IV.
Contraindications
Cardiac arrhythmia, tachycardia >140 bpm, severe HTN, narrow-angle glaucoma, during anesth w/ halogenated hydrocarbon of cyclopropane, local anesth of finger or toe, woman in labor, cardiac dilution & coronary insufficiency.
Special Precautions
Patients w/ CV disease, severe HTN, diabetes, hyperthyroidism, psychoneurosis. Avoid repeated local inj. Potentiated effects w/ diphenhydramine, d-chlorpheniramine & other antihistamines. Pregnancy & lactation. Elderly.
Adverse Reactions
Anxiety, dyspnoea, hyperglycaemia, restlessness, palpitations, tachycardia, tremors, sweating, hypersalivation, weakness, dizziness, headache & coldness of extremities.
