Doxycycline hyclate
Indications/Uses
Used in resp, ENT, GUT, GI, obstetrical & gynecological, ophth & miscellaneous infections.
Dosage/Direction for Use
Adult 2 cap or 200 mg on 1st day in single or divided doses of 1 cap every 12 hr; then 1 cap daily thereafter. Severe infections 1 cap every 12 hr. Acute gonococcal infections 200 mg at once & 100 mg at bedtime on 1st day, followed by 100 mg bid for 3 days. Administer 300 mg at once followed in 1 hr by 2nd 300-mg dose as alternative single visit dose. Primary & secondary syphilis 300 mg daily in divided doses for at least 10 days; continue treatment even after the time that symptoms & fever have subsided. Streptococcal infections Continue therapy for 10 days to prevent rheumatic fever or glomerulonephritis development.
Administration
May be taken with or without food: Take w/ a full glass of water & remain upright for at least 30 min. Take w/ food or milk if GI irritation occurs.
Contraindications
Hypersensitivity. Enamel hypoplasia. Use in tooth development during last ½ of pregnancy, infancy & childhood 8 yr may cause permanent teeth discoloration (yellow-gray-brown).
Special Precautions
Avoid w/ penicillin. Treat all infections due to group A β-hemolytic strep for at least 10 days. Patients on anticoagulant therapy may require downward adjustment of their anticoagulant dosage, due to depressed plasma prothrombin activity. Photosensitivity; discontinue treatment at 1st evidence of skin erythema. May result in overgrowth of non-susceptible organisms including fungi. Superinfection. Dark field exam should be done before treatment is started & blood serology repeated mthly for at least 4 mth in venereal diseases when coexistent w/ syphilis is suspected. False elevations of urinary catecholamine levels w/ fluorescence test. Perform periodic lab evaluation of organ systems, including hematopoietic, renal & hepatic studies in long term therapy. May cause an increase in BUN. Decrease in fibula growth rate in prematures. Pregnancy & lactation. Childn <8 yr.
Adverse Reactions
Anorexia, nausea, vomiting, diarrhea, glossitis, dysphagia, enterocolitis & inflammatory lesions (w/ monilial overgrowth) in the anogenital region; photosensitivity; urticaria, angioneurotic edema, anaphylaxis, anaphylactoid purpura, pericarditis & exacerbation of SLE; bulging fontanels in young infants; hemolytic anemia, thrombocytopenia, neutropenia & eosinophilia.
Drug Interactions
Impaired absorption w/ antacids containing Al, Ca, or Mg, Zn, Fe-containing prep & bismuth subsalicylate. Prolongation of prothrombin time w/ anticoagulants. May interfere w/ bactericidal action of penicillin & aminoglycosides. Serum t½ may be decreased w/ barbiturates, carbamazepine, phenytoin & other hepatic enzyme inducers. May decrease efficacy of contraceptives.
