Doripenem monohydrate
Indications/Uses
Nosocomial pneumonia, complicated intra-abdominal infection, & complicated UTI.
Dosage/Direction for Use
Nosocomial pneumonia including ventilator-associated pneumonia 500 mg every 8 hr by IV infusion for 1 or 4 hr. Complicated intra-abdominal infection 500 mg every 8 hr by IV infusion for 1 hr. Complicated UTI including pyelonephritis 500 mg every 8 hr by IV infusion for 1 hr. Treatment duration: 5-14 days. Moderate renal impairment (CrCl 30-<50 mL/min) 250 mg every 8 hr; severe renal impairment (CrCl <30 mL/min) 250 mg every 12 hr.
Contraindications
Hypersensitivity to doripenem, any other carbapenems, &/or any type of β-lactam antibacterial agent.
Special Precautions
Previous history of hypersensitivity to β-lactam antibiotics. Seizures. Pseudomembranous colitis due to Clostridium difficile. Superinfection may occur; avoid prolonged use. Concomitant use w/ valproic acid/Na valproate is not recommended. Not recommended for patient on any type of dialysis. Pregnancy & lactation. Not recommended for use in childn <18 yr.
Adverse Reactions
Nausea, diarrhea, pruritus, vulvomycotic infection, increased hepatic enzyme, rashes.
Drug Interactions
Reduced serum valproic acid levels leading to inadequate seizure control. Reduced renal clearance w/ probenecid.
