Indications/Uses
Conscious sedation, anesth & sedation in ICU.
Dosage/Direction for Use
IV inj Given slowly at a rate of approx 1 mg in 30 sec. Conscious sedation Adult <60 yr Initially 2.5 mg given 5-10 min before the beginning of the procedure. Further doses of 1 mg may be given as necessary. Adult >60 yr, debilitated or chronically ill patient Reduce initial dose to 1-1.5 mg & given 5-10 min before the beginning of the procedure. Further doses of 0.5-1 mg may be given as necessary. IV induction Each increment of not >5 mg should be inj over 20-30 sec allowing 2 min between successive increments. Premedicated adult <60 yr 0.15-0.2 mg/kg. Non-premedicated adult <60 yr 0.3-0.35 mg/kg. Adult >60 yr, debilitated or chronically ill patient Lower doses will be required. Maintenance 0.03-0.1 mg/kg/hr by either further intermittent doses or continuous IV infusion in combination w/ narcotics or ketamine. IV loading dose Each increment of 1-2.5 mg should be inj over 20-30 sec allowing 2 min between successive increments. Sedation in the ICU Loading dose: 0.03-0.3 mg/kg. Maintenance dose: 0.03-0.2 mg/kg/hr. Hypovolemic, vasoconstricted or hypothermic patient Loading dose: Reduce or omit dose. Maintenance dose: Reduce dose, at times to as low as 25% of the usual dose. IM Premed before induction of anesth Adult <60 yr 0.07-0.1 mg/kg according to the general condition of the patient. Usual dose: 5 mg. Adult >60 yr, debilitated or chronically ill patient 0.025-0.05 mg/kg. Usual dose: 2-3 mg. Childn 1-15 yr 0.08-0.2 mg/kg. Childn receiving ketamine for anesth (ataralgesia) 0.15-0.2 mg/kg.
Contraindications
Hypersensitivity to benzodiazepines. Conscious sedation in patients w/ severe resp failure or acute resp depression.
Special Precautions
Severe cardiorespiratory adverse events including resp depression, apnoea, resp arrest &/or cardiac arrest w/ rapid inj or when high dosage is administered. Conscious sedation in patients w/ impaired resp function. Not to be given during pregnancy & lactation.
Adverse Reactions
Slight decrease in arterial BP & changes of heart rate & respiration. Nausea, vomiting, headache, hiccups, laryngospasm, dyspnea, hallucination, over sedation, drowsiness & ataxia; local effects on veins (pain on inj & thrombophlebitis); w/drawal symptoms after abrupt discontinuation of prolonged IV administration.
Drug Interactions
Enhancement of the central depressive effect w/ antipsychotics (neuroleptics), hypnotics, anxiolytics/sedatives, antidepressants, narcotic analgesics, antiepileptic drugs, anesth & sedative antihistamines. Prolonged sedation w/ ketoconazole, itraconazole, erythromycin, diltiazem, verapamil, cimetidine & ranitidine (tab). May enhance sedative effect w/ alcohol.
