Methyldopa sesquihydrate
Indications/Uses
Dosage/Direction for Use
Adult Initially 250 mg bid-tid for 2 days. Max: 3 g daily. Maintenance: 500 mg-2 g daily. Elderly Initially 125 mg bid, may be increased gradually but not more than 2 g daily.
Administration
May be taken with or without food.
Special Precautions
Caution in patients w/ impaired renal or hepatic function, history of hemolytic anemia, liver disease or depression. Do not use in patients w/ active liver disease or depression. Not recommended for pheochromocytoma. May produce +ve response to a direct Coombs' test reaction. May cause sedation. Patient is not advisable to drive or operate machinery.
Adverse Reactions
Drowsiness; dizziness & lightheadedness associated w/ postural hypotension; fluid retention & oedema; angina pectoris, bradycardia, syncope, prolonged carotid sinus hypersensitivity. Impaired conc & memory, mild psychoses, depression, disturbed sleep & nightmares, paraesthesias, Bell's palsy, involuntary choreoathetotic movements & parkinsonism. GI disturbances eg, nausea & vomiting, diarrhea, constipation. Black or sore tongue, inflammation of salivary glands, dry mouth. Myocarditis, fever, eosinophilia, liver function disturbances, rashes, lichenoid & granulomatous eruptions, toxic epidermal necrolysis, flu-like syndrome, nocturia, uraemia, nasal congestion & retroperitoneal fibrosis. Hyperprolactinaemia w/ gynaecomastia, galactorrhea & amenorrhea. Hepatitis, antinuclear antibodies may develop, lupus-like syndrome.
Drug Interactions
Potentiated hypotensive effects w/ diuretics, other antihypertensives & drug w/ hypotensive effects. Paradoxical antagonism w/ TCAs, antipsychotics, β-blockers, sympathomimetics. Concomitant use w/ MAOIs. Potential metabolism inhibition w/ entacapone. May require lower doses of general anaesth in concomitant use.
