Indications/Uses
Prevention & treatment of vit D deficiency. Adjunct to specific therapy for osteoporosis in patients w/ vit D deficiency or at risk of vit D insufficiency.
Dosage/Direction for Use
Prevention of vit D deficiency Adult 25,000 IU/mth (1 amp), higher doses may be required. Childn 1-18 yr 25,000 IU (1 amp) every 6 wk, 0-1 yr 25,000 IU (1 amp) every 8 wk. Treatment of vit D deficiency (<25 ng/mL) Adult 50,000 IU/wk (2 amp) for 6-8 wk, followed by maintenance therapy 1,400-2,000 IU/day. Childn 0-18 yr 25,000 IU (1 amp) once every 2 wk for 6 wk, followed by maintenance therapy of 400-1,000 IU daily. Adjunct to specific therapy for osteoporosis Adult 25,000 IU/mth (1 amp).
Administration
Should be taken with food: Take full contents of the amp emptied into the mouth then swallow, or emptied onto a spoon then take orally. May also mix w/ a small amount of cold or lukewarm food prior intake.
Contraindications
Hypersensitivity. Hypercalcemia &/or hypercalciuria; nephrolithiasis &/or nephrocalcinosis; hypervitaminosis D; pseudohypoparathyroidism. Serious renal impairment.
Special Precautions
Monitor Ca & phosphate levels of patients w/ renal impairment. Patients receiving treatment for CV disease. Monitor serum & urinary Ca levels in patients w/ sarcoidosis. Risk of renal stones w/ concomitant Ca supplementation. Increased risk of fractures in elderly (greatest increase occurring during the 1st 3 mth after dosing). Pregnancy & lactation (on high strength formulation).
Drug Interactions
May reduce effect by metabolic inactivation w/ anticonvulsants (eg, phenytoin) or barbiturates. Decreased urinary elimination of Ca w/ thiazide diuretics. Decreased effect w/ glucocorticoids. May increase risk of digitalis toxicity (arrhythmia) w/ drugs containing digitalis & other cardiac glycosides. May reduce GI absorption w/ ion exchange resin (eg, cholestyramine, colestipol HCl, orlistat) or laxative (eg, paraffin oil). Interfered vit D activity w/ actinomycin & imidazole antifungal agents.
