Losartan K
Indications/Uses
HTN. Reduction in the risk of CV morbidity & mortality in hypertensive patients w/ left ventricular hypertrophy. Renal protection in type 2 diabetic patients w/ proteinuria.
Dosage/Direction for Use
HTN 50 mg once daily, may be increased to 100 mg once daily. Patient w/ intravascular vol-depletion 25 mg once daily. Reduction in the risk of CV morbidity & mortality in hypertensive patient w/ left ventricular hypertrophy 50 mg once daily. A low dose of hydrochlorothiazide should be added &/or the dose of losartan K should be increased to 100 mg once daily based on BP response. Renal protection in type 2 diabetic patient w/ proteinuria 50 mg once daily, may be increased to 100 mg once daily based on BP response.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Co-administration of aliskiren in patients w/ diabetes.
Special Precautions
Symptomatic hypotension may occur in patients who are intravascularly vol-depleted (eg, those treated w/ high-dose diuretics). Electrolyte imbalances. Concomitant use of other drugs that may increase K may lead to hyperkalemia. History of hepatic impairment. Other drugs that affect RAAS may increase blood urea & serum creatinine in patients w/ bilateral renal artery stenosis or stenosis of the artery to a solitary kidney. Avoid use of aliskiren in patients w/ renal impairment (GFR <60 mL/min). Discontinue therapy as soon as pregnancy is detected. Lactation. Neonates w/ history of in utero exposure to losartan. Not recommended in ped patients w/ GFR <30 mL/min/1.73 m2, those w/ hepatic impairment, or in neonates. Black patients.
Adverse Reactions
Dizziness, asthenia/fatigue, & vertigo. Abdominal pain, chest pain, edema/swelling; palpitation, tachycardia; diarrhea, dyspepsia, nausea; back pain, muscle cramps; headache, insomnia; cough, nasal congestion; pharyngitis, sinus disorder, URTI. Hypotension, hyperkalemia.
Drug Interactions
Reduced levels of active metabolite w/ rifampicin & fluconazole. Increase in serum K w/ K-sparing diuretics (eg, spironolactone, triamterene, amiloride), K supplements, K-containing salt substitutes or other drugs that may increase serum K (eg, trimethoprim-containing products). May reduce lithium excretion. May reduce effect w/ NSAIDs including selective COX-2 inhibitors. Concomitant use w/ NSAIDs may result in further deterioration of renal function including possible acute renal failure in patients w/ compromised renal function (eg, elderly or vol-depleted patients including those on diuretic therapy). Increased risks of hypotension, syncope, hyperkalemia & changes in renal function w/ angiotensin receptor blockers, ACE inhibitors & aliskiren. Reduced therapeutic effect w/ grapefruit juice.
