Losartan K 50 mg, hydrochlorothiazide 12.5 mg
Indications/Uses
Management of HTN. Losartan K: Diabetic nephropathy; patients who develop cough w/ ACE inhibitors; reduce risk of stroke in patients w/ left ventricular hypertrophy. Hydrochlorothiazide: Edema associated w/ heart failure & renal hepatic disorders; edema accompanying premenstrual syndrome; diabetes insipidus. Prevention of water retention-associated w/ corticosteroids & estrogens; renal calculus formation.
Dosage/Direction for Use
1 tab once daily, may be increased to 2 tab as a single or in 2 divided doses. Elderly >75 yr, moderate to severe renal impairment (CrCl <20 mL/min), intravascular fluid depletion Initially 25 mg once daily.
Administration
May be taken with or without food.
Contraindications
Severe renal impairment (CrCl <30 mL/min) or anuria; Addison's disease; preexisting hypercalcemia. Pregnancy & lactation.
Special Precautions
Renal artery stenosis or renal impairment. Observe for signs of fluid & electrolyte imbalance especially in the presence of vomiting or during parenteral fluid therapy.
Adverse Reactions
Losartan K: Dizziness, dose-related orthostatic hypotension; hyperkalemia in renal disease; resp tract disorder, back pain, GI disturbances, fatigue, neutropenia. Hydrochlorothiazide: Hyperglycemia & glycosuria; hyperuricemia & gout attacks; electrolyte imbalances eg, hypochloremic alkalosis, hyponatremia, hypokalemia.
Drug Interactions
Losartan K: Reduced active metabolite levels w/ rifampicin & fluconazole. Increased serum K levels w/ K-sparing diuretics, K supplements or K-containing salt substitutes. Hydrochlorothiazide: Potentiated orthostatic hypotension w/ alcohol, barbiturates or narcotics. Oral antidiabetic drugs & insulin. Additive effect w/ other antihypertensives. Impaired absorption w/ cholestyramine & colestipol resins (anionic resin exchange). Intensified electrolyte depletion w/ corticosteroids, ACTH. Possible decreased response w/ pressor amines (eg, adrenaline).
