Zuclopenthixol acetate
Indications/Uses
Initial treatment of acute psychoses, including mania & exacerbation of chronic psychoses.
Dosage/Direction for Use
IM Adult 50-150 mg (1-3 mL) repeated if necessary w/ 2-3 days interval. May inj 24-48 hr following 1st inj. Max: 400 mg for 4 inj & should not be >2 wk. Elderly Max: 100 mg.
Contraindications
Hypersensitivity. Circulatory collapse, depressed consciousness level due to any cause (eg, alcohol, barbiturates or opiate intoxication), coma.
Special Precautions
Possible development of neuroleptic malignant syndrome. Patients w/ preexisting organic brain syndrome, mental retardation, opiate & alcohol abuse; convulsion & advanced hepatic disease; hypokalemia, hypomagnesemia or genetic predisposition; & history of CV disorders. May modify insulin & glucose response. May cause QT prolongation. Venous thromboembolism. Avoid concomitant use w/ other antipsychotics. Determine serum liver function. Not use for treatment of dementia-related behavioral disturbances. May impair ability to drive or operate machinery. Pregnancy & lactation. Not recommended for childn. Elderly (w/ dementia & risk for cerebrovascular adverse events).
Adverse Reactions
Somnolence, akathisia, hyperkinesia & hypokinesia; dry mouth. Increased appetite & wt; insomnia, depression, anxiety, nervousness, abnormal dreams, agitation, decreased libido; tremor, dystonia, hypertonia, dizziness, headache, paraesthesia, attention disturbance, amnesia, abnormal gait; accommodation disorder, abnormal vision; vertigo; tachycardia, palpitations; nasal congestion, dyspnea; salivary hypersecretion, constipation, vomiting, dyspepsia, diarrhea; hyperhidrosis, pruritus; myalgia; micturition disorder, urinary retention, polyuria; asthenia, fatigue, malaise, pain.
Drug Interactions
May enhance sedative effects of alcohol, barbiturates & other CNS depressants. May increase or reduce anti-hypertensive effect of guanethidine & similar acting compd. Increased risk of neurotoxicity w/ lithium. Mutual metabolism inhibition w/ TCAs. May reduce effect of levodopa & adrenergic drugs. Increased risk of extrapyramidal disorder w/ metoclopramide & piperazine. Decreased clearance w/ CYP2D6 inhibitors. Increased QT interval w/ class Ia & III antiarrhythmics (eg, quinidine, amiodarone, sotalol, dofetilide), some antipsychotics (eg, thioridazine), some macrolides (eg, erythromycin), some antihistamines (eg, terfenadine, astemizole), some quinolone antibiotics (eg, gatifloxacin, moxifloxacin) & some other drugs (eg, cisapride & lithium). Increased risk of QT prolongation & malignant arrhythmias eg, thiazide diuretics.
