Co-amoxiclav: Amoxicillin 500 mg, clavulanic acid 125 mg
Indications/Uses
Infections due to β-lactamases-producing strains (where amoxicillin alone is not appropriate) eg, resp tract, genitourinary & abdominal infections, cellulitis, animal bites, severe dental infections w/ spreading cellulitis.
Dosage/Direction for Use
Adult 625 mg tab every 12 hr or 375 mg tab every 8 hr. More severe infections & resp tract infections 1,000 mg tab every 12 hr or 625 mg tab every 8 hr. Severely impaired patient w/ GFR 10-30 mL/min 625 mg or 375 mg every 12 hr, <10 mL/min 625 mg or 375 mg every 24 hr. Ped patient weighing ≥40 kg Adult dose.
Administration
May be taken with or without food: Best taken at the start of meals for better absorption & to reduce GI discomfort.
Contraindications
Hypersensitivity to amoxicillin, clavulanic acid, or β-lactams (eg, penicillins or cephalosporins). Patients w/ previous history of Co-Amoxiclav associated jaundice/hepatic dysfunction.
Special Precautions
Should not be used w/ drugs that delay peristalsis (eg, opiates & diphenoxylate w/ atropine). Fluids, electrolytes & protein replacement therapy should be provided when indicated. Patients w/ evidence of hepatic dysfunction. Avoid during pregnancy (especially 1st trimester). Lactation.
Adverse Reactions
Gastritis, stomatitis, glossitis, black hairy tongue, indigestions, nausea, vomiting, diarrhea, enterocolitis, pseudomembranous colitis, candidiasis; skin rashes, urticaria; serious & occasional fatal hypersensitivity (anaphylactic) reactions, angioneurotic edema; cross-sensitivity w/ other β-lactams; transient hepatitis, cholestatic jaundice; anemia, thrombocytopenia, thrombocytopenic purpura, eosinophilia, leukopenia & agranulocytosis.
Drug Interactions
May prolong or worsen the condition w/ drugs that delay peristalsis (eg, opiates & diphenoxylate w/ atropine). Transient decrease in plasma conc of total conjugated estriol, cestadiol in pregnant women. Amoxicillin: Decreased renal tubular secretion w/ probenecid. Disulfiram (antabuse) like reaction w/ alcohol.
