Azithromycin dihydrate
Indications/Uses
Cinazith Infection from resp tract pathogens, uncomplicated chlamydial urethritis, cervicitis or pharyngitis. Alternative drug for multidrug resistant Salmonella typhi infection outside the CNS. Cinazith-DS Resp tract infection (including otitis media) in skin & soft tissue; uncomplicated genital infections. Prophylaxis & as component of regimens in the treatment of M. avium complex (MAC) infections.
Dosage/Direction for Use
Cinazith Adult & adolescent 1 g as a single dose. Gonococcal infection Adult & adolescent 2 g as a single dose. Otitis media & pneumonia Childn 6 mth-12 yr 10 mg/kg once a day on the 1st day, then 5 mg/kg once a day on days 2-5. Strep throat Adult & adolescent ≥16 yr 1,200 mg once a wk. Childn 2-12 yr 12 mg/kg once a day for 5 days. Bronchitis, strep throat, pneumonia & skin infection Adult & adolescent ≥16 yr 500 mg on the 1st day, then 250 mg once a day on days 2-5. Childn ≥16 yr 1,000 mg as a single dose. Sinusitis Adult & adolescent 500 mg for 3 days. Chlamydia Adult & adolescent ≥16 yr 1,000 mg as a single dose. Cinazith-DS Childn <16 yr 10 mg/kg once daily for 3 days. Alternatively, after 10 mg/kg loading dose on the 1st day, may be continued as 5 mg/kg for the next 4 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to azithromycin, macrolides or ketolides.
Special Precautions
Do not use in patients w/ hepatic impairment. Monitor cyclosporine levels before concurrent administration w/ cyclosporine. Hepatic impairment. Pregnancy & lactation. Infants ≤6 mth. Childn & elderly.
Adverse Reactions
Nausea, abdominal pain, diarrhea. Skin rash. Stomach cramps or pain (severe); abdominal tenderness; diarrhea (watery & severe, which may be bloody); difficulty in breathing; fever; joint pain; swelling of face, mouth, neck, hands & feet. Vomiting. Occasionally reversible elevations in liver transaminases.
Drug Interactions
May cause elevated digoxin conc. Liver enzyme abnormalities & hearing impairment w/ nelfinavir. Increased anticoagulant effects w/ warfarin. Severe peripheral vasospasm & dysesthesia w/ ergotamine & dihydroergotamine. Possible ergotism w/ ergot derivatives. Reduced peak serum conc w/ antacids. Caution when in concurrent use w/ cyclosporine.
