Cefuroxime (Tab: axetil; Powd for inj: Na)
Indications/Uses
Tab: Infections due to sensitive gm +ve & gm -ve bacteria & has greater activity against H. influenzae & Neisseria gonorrhoeae. Powd for inj: Alternative drug for infections due to susceptible organisms eg, in otitis media, orbital cellulitis; urinary tract, skin & soft tissue, bone & joint infections & post-splenectomy sepsis of unclear etiology.
Dosage/Direction for Use
Tab Adult & adolescent ≥13 yr Pharyngitis/tonsillitis, acute bacterial maxillary sinusitis 250 mg bid for 10 days. Acute bacterial exacerbations of chronic bronchitis 250 or 500 mg bid for 10 days. Secondary bacterial infection of acute bronchitis 250 or 500 mg bid for 5-10 days. Uncomplicated urinary tract, skin & skin structure infections 250 mg bid for 7-10 days. Uncomplicated gonorrhea 1,000 mg once w/ single dose probenecid 1,000 mg. Early Lyme disease 500 mg bid for 20 days. Childn Acute otitis media & bacterial maxillary sinusitis 250 mg bid for 10 days. Powd for inj: IM/IV Adult 750 mg to 1.5 g 8 hrly for 5-10 days. Uncomplicated UTI, skin structure infections, disseminated gonococcal infections & uncomplicated pneumonia 750 mg 8 hrly. Severe & complicated infections, bone & joint infections 1.5 g 8 hrly. Severe infections or bacterial meningitis 1.5 g 6 hrly not exceeding 3 g 8 hrly. Uncomplicated gonococcal infection 1.5 g IM as single dose + oral probenecid 1 g. Childn 50-100 mg/kg/day. Bone & joint infections 150 mg/kg/day. Bacterial meningitis 200-240 mg/kg/day in equally divided doses 6-8 hrly.
Administration
Should be taken with food: Take w/ meals for optimum absorption.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
Prolonged use. Pregnancy. Tab: History of colitis. Concurrent use w/ potent diuretics. Monitor prothrombin time in patients w/ renal/hepatic impairment of poor nutritional state, those receiving protracted course of antimicrobial therapy, previously stabilized on anticoagulant therapy. Consider CDAD if diarrhea occurs. Lactation. Powd for inj: Hypersensitivity to penicillins. Anaphylactic shock. Pronounced renal insufficiency. False +ve reaction/result of glycosuria using Benedict's, Fehling's & Clinitest & in direct Coombs' test. Childn <3 mth.
Adverse Reactions
Tab: Anaphylaxis, angioedema, pruritus, rash, serum sickness-like reaction, urticaria, pseudomembranous colitis; hemolytic anemia, leukopenia, pancytopenia, thrombocytopenia, increased prothrombin time, hepatic impairment including hepatitis & cholestasis, jaundice, seizure, erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis, renal dysfunction; diarrhea/loose, nausea/vomiting, transient elevation in AST, ALT & lactate dehydrogenase, eosinophilia, abdominal pain & cramps, flatulence, indigestion, headache, vaginitis, vulvar itch, hives, itch, dysuria, chills, chest pain, shortness of breath, mouth ulcers, swollen tongue, sleepiness, thirst, anorexia, +ve Coombs' test & mild to moderate hearing loss. Powd for inj: Glossitis, nausea, vomiting, gastric pyrosis, abdominal pain & diarrhea.
Drug Interactions
Probenecid; drugs that reduce gastric acidity; oral estrogen/progesterone contraceptives.
