Indications/Uses
Treatment & prophylaxis of anaerobic & mixed bacterial infections especially intra-abdominal & pelvic infections. Gonorrhea & UTI; pelvic inflammatory disease. Prophylaxis of endometritis at caesarian section & surgical infection.
Dosage/Direction for Use
Dose may be given by deep IM inj, slow IV inj over 3-5 min, or by continuous IV infusion. Adult Usual dose: 1-2 g every 8 hr or every 4 or 6 hr. Severe infections Up to 12 g daily. Uncomplicated UTI 1 g IM bid. Uncomplicated gonorrhoea 2 g IM as a single dose w/ probenecid 1 g PO. Surgical infection prophylaxis 2 g IM/IV 30-60 min prior to the procedure & then every 6 hr not usually for >24 hr. Caesarean section 2 g IV as a single dose given as soon as umbilical cord is clamped. A 3-dose regimen w/ further 2-g doses 4 & 8 hr after initial dose may be used if necessary. Childn & older infant 20-40 mg/kg every 6-8 hr. Surgical infection prophylaxis 30-40 mg/kg 30-60 min prior to the procedure & then every 6 hr not usually for >24 hr. Neonate 1-4 wk 20-40 mg/kg every 8 hr, up to 1 wk 20-40 mg/kg every 12 hr. Surgical infection prophylaxis 30-40 mg/kg every 8-12 hr. Renal impairment Loading dose: Initially 1-2 g. Maintenance dose: CrCl 30-50 mL/min 1-2 g every 8-12 hr, 10-29 mL/min 1-2 g every 12-24 hr, 5-9 mL/min 500 mg-1 g every 12-24 hr, <5 mL/min 500 mg-1 g every 24-48 hr. Patient undergoing hemodialysis Repeat loading dose after each dialysis session.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
May interfere w/ Jaffe method to reduce falsely high values. May have +ve response to Coombs' test. Lactation.
Adverse Reactions
Hypersensitivity reactions, including skin rashes, urticaria, eosinophilia, fever, serum sickness-like reactions, anaphylaxis. Neutropenia, thrombocytopenia. Bleeding complications related to hypoprothrombinaemia &/or platelet dysfunction. Marked changes in anaerobic, facultative & aerobic fecal flora.
Drug Interactions
Reduced renal clearance w/ probenecid.
