Indications/Uses
Lower resp tract, gynecological, skin & skin structure, intra-abdominal, bone or joint, & CNS infections, UTI; bacteremia/septicemia. Peri-op prophylaxis.
Dosage/Direction for Use
Deep IM inj or slow IV inj over 3-5 min or by infusion over 20-60 min. Adult 2-6 g daily in 2-4 divided doses. Severe infections Up to 12 g daily by IV in up to 6 divided doses. Pseudomonal infections >6 g daily. Gonorrhea 500 mg or 1 g as single dose. Surgical infection prophylaxis 1 g for 30-90 min before surgery. Caesarean section 1 g IV given after the umbilical cord is clamped, then 2 further doses IM/IV 6 & 12 hr later. Childn 100-150 mg/kg daily in 2-4 divided doses. Severe infections 200 mg/kg daily. Neonate 50 mg/kg daily in 2-4 divided doses. Severe infections 150-200 mg/kg daily. Severe renal impairment Initial loading dose of 1 g then halving the dose while maintaining usual frequency of dosing.
Contraindications
History of allergy or hypersensitivity to cephalosporins.
Special Precautions
Penicillin-allergic patients. Avoid alcohol or alcohol-containing medications. Pregnancy & lactation.
Adverse Reactions
Hypersensitivity reactions, including skin rashes, urticaria, eosinophilia, fever, serum sickness-like reactions, & anaphylaxis. Neutropenia & thrombocytopenia. Transient increase in liver enzyme values. Convulsions & other signs of CNS toxicity (high doses). Pseudomembranous colitis (prolonged use). Pain at inj site (IM use) & thrombophlebitis (IV infusion).
Drug Interactions
Increased nephrotoxicity w/ probenecid, ethacrynic acid, furosemide, gentamicin & vancomycin. Disulfiram-like reaction w/ alcohol ingestion. May increase risk of hemorrhage w/ high-dose heparin, oral anticoagulants & other drugs affecting hemostasis.
