HTN Initially 12.5 mg once daily or 6.25 mg bid, may be increased at 2-wk intervals. Max: 50 mg.
Angina pectoris Initially 12.5 mg bid for 2 days then increase to 25 mg bid. Dose may be increased at 2-wk intervals. Max: 100 mg bid.
CHF Initially 3.125 mg bid for 2 wk. If dose is tolerated, increase to 6.25 mg bid, followed by 12.5 mg bid & thereafter 25 mg bid at 2-wk intervals.
Mild to moderate heart failure in patients >85 kg Max: 50 mg bid.
Severe heart failure & mild to moderate heart failure in patient <85 kg Max: 25 mg bid.
Left ventricular dysfunction following MI Initially 6.25 mg, then increase to bid & maintain for 3-10 days. Decrease dose to 3.125 mg bid if patient develop signs of tolerance. Max: 25 mg bid.
