Indications/Uses
Short-term treatment of HTN when oral therapy is not feasible or not desirable.
Dosage/Direction for Use
IV Infusion rate: Initially 5 mg/hr, increased as necessary up to max 15 mg/hr & subsequently reduced to 3 mg/hr.
Contraindications
Hypersensitivity. Patients w/ advanced aortic stenosis, compensatory HTN (ie, in case of arteriovenous shunt or aortic coarctation), unstable angina, w/in 8 days after MI.
Special Precautions
Patients w/ acute cerebral infarction or hemorrhage & systemic hypotension. CHF, especially those receiving concomitant β-adrenergic blocking agents. May increase frequency, duration, & severity of angina during initiation of therapy or upward adjustment of dosage. Peripheral edema. Avoid abrupt w/drawal. If initiated in patients currently receiving β-adrenergic blocking agent w/ the intent to w/draw β-adrenergic blocking agent should gradually reduce dosage, preferably 8-10 days. Use w/ caution & in reduced dosage in patients w/ hepatic impairment or reduced hepatic blood flow. Increased bioavailability & elimination t½ in patients w/ severe hepatic impairment. Portal HTN. Pregnancy.
Adverse Reactions
Dizziness, flushing, headache, hypotension, peripheral oedema, tachycardia, & palpitations. Nausea & other GI disturbances, increased micturition frequency, lethargy, eye pain, visual disturbances, & mental depression. Paradoxical increase in ischaemic chest pain & excessive fall in BP leading to cerebral or myocardial ishcaemia or transient blindness. Rashes (including erythema multiforme), fever & abnormalities in liver function including cholestasis. Gingival hyperplasia, myalgia, tremor & impotence.
