Insuline glargine
Indications/Uses
Type 1 DM in adult & childn. Type 2 DM in adult.
Dosage/Direction for Use
Individualized dosage. Administer SC once daily at the same time everyday. Type 1 diabetes Initially approx 1/3 of the total daily insulin requirements. Patients w/ type 2 diabetes who are currently treated w/ insulin Initially 10 u (or 0.2 u/kg) once daily. May be adjusted according to blood glucose measurement.
Contraindications
Special Precautions
Do not administer IV or via insulin pump. Hypoglycemia. Severe, life-threatening, generalized allergy including anaphylaxis. Fluid retention & heart failure w/ concomitant use w/ PPAR-γ-agonists eg, thiazolidinediones. Renal & hepatic impairment. May impair ability to drive or operate machinery. Pregnancy & lactation. Childn w/ type 2 diabetes & childn <6 yr w/ type 1 diabetes. Elderly.
Adverse Reactions
Hypoglycemia; lipohypertrophy, inj site reactions eg, redness, pain, itching, swelling or inflammation.
Drug Interactions
Increased blood glucose-lowering effects w/ oral antidiabetics, pramlintide, ACE inhibitors, disopyramide, fibrates, fluoxetine, MAOIs, propoxyphene, pentoxifylline, salicylates, somatostatin analogs & sulfonamides. Reduced blood-glucose-lowering effects w/ corticosteroids, niacin, danazol, diuretics, sympathomimetics (eg, epinephrine, albuterol, terbutaline), glucagon, INH, phenothiazine derivatives, somatropin, thyroid hormones, estrogens, progestogens (eg, OCs), PIs & atypical antipsychotics (eg, olanzapine & clozapine). Potentiated or weaken blood glucose-lowering effects w/ β-blockers, clonidine, lithium salts & alcohol. May cause hypoglycemia w/ pentamidine. Signs of hypoglycemia may be reduced or absent w/ β-blockers, clonidine, guanethidine, reserpine.
