Indications/Uses
Short-term treatment (7-10 days) of adult patients w/ GERD & history of erosive esophagitis. Pathological hypersecretory conditions including Zollinger-Ellison syndrome in adults.
Dosage/Direction for Use
IV Adult GERD w/ history of erosive esophagitis 40 mg once daily by infusion for 7-10 days. Pathological hypersecretion including Zollinger-Ellison syndrome 80 mg every 12 hr. Max: 240 mg daily.
Contraindications
Special Precautions
Symptomatic response does not preclude the presence of gastric malignancy. Anaphylaxis & other serious reactions eg, erythema multiforme, SJS & TEN. Thrombophlebitis. Increased risk of C. difficile-associated diarrhea, osteoporosis-related fractures of the hip, wrist or spine. Mild, transient transaminase elevations; symptomatic & asymptomatic hypomagnesemia. Concomitant use w/ MTX. Hepatic impairment (>40 mg/day). False +ve result w/ urine screen for tetrahydrocannabinol. Pregnancy & lactation.
Adverse Reactions
Asthenia, fatigue, malaise; anaphylaxis; wt changes; severe dermatologic reactions including, erythema multiforme, Stevens-Johnson syndrome & TEN & angioedema (Quincke's edema); rhabdomyolysis, bone fracture; interstitial nephritis; hepatocellular damage leading to jaundice & hepatic failure; hallucinations, confusion, insomnia, somnolence; hyponatremia, hypomagnesemia, C. difficile-associated diarrhea; pancytopenia, agranulocytosis; ageusia, dysgeusia.
Drug Interactions
May decrease atazanavir or nelfinavir plasma conc. Increased INR & prothrombin time w/ warfarin. May interfere w/ absorption of drugs where gastric pH is important determinant of their bioavailability (eg, ketoconazole, ampicillin, esters, Fe salts & digoxin). May elevate & prolong serum levels of MTX.
