Indications/Uses
Hyperlipidemias including hypercholesterolemia & combined hyperlipidemia (type IIa or IIb hyperlipoproteinemias), hypertriglyceridemia (type IV) & dysbetaliproproteinemia (type III). Adjunctive therapy in patients w/ HoFH. Primary prophylaxis of CV events.
Dosage/Direction for Use
Initially 10-20 mg once daily. Patient who require large reduction in LDL-C 40 mg daily. May be adjusted at 4-wk interval. Max: 80 mg daily. Childn & adolescent 10-17 yr Heterozygous familial hypercholesterolemia 10 mg once daily. Max: 20 mg once daily.
Administration
May be taken with or without food: Avoid excessive consumption (>1 L/day) of grapefruit juice.
Contraindications
Hypersensitivity. Active liver disease or unexplained persistent raised serum aminotransferase conc. Women of childbearing potential. Pregnancy & lactation.
Special Precautions
Potential effects of statins on growth & sexual development in childn prior to lifelong therapy. Porphyria.
Adverse Reactions
GI disturbances. Headache, skin rashes, dizziness, blurred vision, insomnia, dysgeusia. Hepatitis & pancreatitis. Hypersensitivity reactions including anaphylaxis & angioedema have also occurred. Myopathy, characterized by myalgia & muscle weakness & associated w/ increased creatine phosphokinase conc.
Drug Interactions
Increased risk of myopathy w/ fibric acid derivatives (fibrates or gemfibrozil) & nicotinic acid. Ciclosporin, itraconazole, ketoconazole, erythromycin, clarithromycin, HIV-PIs, nefazodone, amiodarone, verapamil & grapefruit juice. Bleeding & increases in prothrombin time w/ coumarin anticoagulants. Increased plasma conc w/ erythromycin & other macrolides, itraconazole, ketoconazole, HIV-PIs & grapefruit juice. Reduced plasma conc w/ rifampicin. Risk for rhabdomyolysis & hepatitis w/ diltiazem. Reduction in 1st-pass metabolism due to inhibition of P-gp by esomeprazole.
