Indications/Uses
Signs & symptoms of acute OA & RA. Management of acute pain associated w/ inflammation.
Dosage/Direction for Use
OA 30 mg once daily, increased to 120 mg once daily if necessary. RA 90 mg once daily. Acute gouty arthritis 120 mg once daily, max of 8 days.
Administration
May be taken with or without food.
Contraindications
Stroke; MI; CABG; uncontrolled HTN; CHF NYHA II-IV.
Special Precautions
Not to be given to patients w/ allergy to NSAIDs & asthma. Patients w/ ischemic heart disease, risk factors for heart disease, HTN, hyperlipidemia, diabetes, smoking & peripheral arterial disease. Use the lowest effective dose for the shortest duration of treatment in patients w/ CV risk. Discontinue use if appearance of skin rash & signs of hypersensitivity occurs. Potential GI (gastric & liver) & renal toxicities. Not to be used in patients w/ HTN whose BP is inadequately controlled. Contraindicated in patients w/ inflammatory bowel disease, moderate to severe heart failure & renal impairment (CrCl <30 mL/min). May affect ability to drive & use machines. Not recommended in women attempting to conceive. Contraindicated in pregnancy. Lactation.
Adverse Reactions
GI discomfort, nausea, & diarrhea; peptic ulceration & severe GI bleeding. Headache, vertigo, dizziness, nervousness, tinnitus, depression, drowsiness, & insomnia. Fever, angioedema, bronchospasm, & rashes. Hepatotoxicity & aseptic meningitis. Visual disturbance. Anemia, thrombocytopenia, neutropenia, eosinophilia, & agranulocytosis. Interstitial nephritis & nephrotic syndrome; renal failure. Hematuria. Nephropathy. Fluid retention. Photosensitivity, alveolitis, pulmonary eosinophilia, pancreatitis, SJS & TENs. Induction or exacerbation of colitis.
Drug Interactions
Changes in plasma conc w/ drugs that inhibit or induce CYP450 isoenzyme CYP3A4. Decreased plasma conc w/ rifampicin. Increased plasma conc of ethinylestradiol. Oral salbutamol & minoxidil.
