Indications/Uses
Short-term treatment of severe anxiety disorders & insomnia; sedative & premedicant; management of status epilepticus & febrile convulsions, in the control of muscle spasm as in tetanus; management of alcohol w/drawal symptoms.
Dosage/Direction for Use
Severe anxiety Up to 10 mg IM/IV repeated after 4 hr, if necessary. Premed before general anesth 100-200 mcg/kg. Sedation during minor surgical & medical procedures 10-20 mg IV over 2-4 min. Seizures Adult 10-20 mg at a rate 5 mg/min repeated after 30-60 min if necessary. Childn 200-300 mcg/kg. Muscle spasm 10 mg repeated after 4 hr if necessary. Tetanus Adult & childn 100-300 mcg/kg IV every 1-4 hr. Alcohol w/drawal syndrome 10-20 mg IM/IV. Elderly & debilitated patient ½ adult dose.
Contraindications
Preexisting CNS depression or coma, resp depression, acute pulmonary insufficiency, sleep apnea.
Special Precautions
Chronic pulmonary insufficiency; muscle weakness; impaired liver or kidney function. Monitor cardioresp function. Personality disorders; organic brain changes particularly atherosclerosis. History of alcohol or drug addiction. Development of dependence after regular use. May impair ability to drive or operate machinery.
Adverse Reactions
Drowsiness, sedation, muscle weakness & ataxia.
Drug Interactions
CNS depressants, cisapride, analgesics, antibacterials, anticoagulants, antidepressants, antiepileptics, antivirals, β-blockers, clozapine, digoxin, disulfiram, cimetidine, omeprazole, levodopa, neuromuscular blockers, OCs, penicillamine, papaverine, tobacco, xanthines.
