Cefepime HCl
Indications/Uses
Infections due to susceptible organisms including urinary tract, resp tract & skin infections.
Dosage/Direction for Use
Adult (CrCl >60 mL/min) Moderate to severe pneumonia 1-2 g IV every 12 hr for 10 days. Empiric therapy for febrile neutropenic 2 g IV every 8 hr for 7 days. Mild to moderate uncomplicated or complicated UTI including pyelonephritis 0.5-1 g IV/IM every 12 hr for 7-10 days. Severe uncomplicated UTI including pyelonephritis 2 g IV every 12 hr for 10 days. Moderate to severe uncomplicated skin & skin structure infections 2 g IV every 12 hr for 10 days. Complicated intra-abdominal infections 2 g IV every 12 hr for 7-10 days. Childn 2 mth up to 16 yr (up to 40 kg) Uncomplicated & complicated urinary tract, uncomplicated skin & skin structure infection & pneumonia 50 mg/kg/dose every 12 hr. Febrile neutropenic 50 mg/kg/dose every 8 hr. Max: Adult dose.
Contraindications
Hypersensitivity to cefepime or cephalosporins, penicillins or other β-lactam.
Special Precautions
Clostridium difficile-associated diarrhea (CDAD). Overgrowth of non-susceptible microorganisms (prolonged use). Patients w/ renal (especially elderly) or hepatic impairment, poor nutritional state & currently receiving protracted course of antimicrobial therapy; history of GI disease & colitis. Monitor prothrombin time. +ve result for Coombs' test; false +ve reaction in urine using Clinitest. Monitor signs & symptoms of neurological adverse reactions prior to therapy. Pregnancy & lactation. Infant <2 mth.
Adverse Reactions
Local reactions including phlebitis, pain &/or inflammation; rash. Colitis (including pseudomembranous colitis), diarrhea, erythema, fever, headache, nausea, oral moniliasis, pruritis, urticaria, vaginitis & vomiting.
Drug Interactions
Increased potential nephrotoxicity & ototoxicity of aminoglycosides; nephrotoxicity w/ diuretics (eg, furosemide).
