Indications/Uses
Maintenance treatment of reversible bronchospasm associated w/ bronchial asthma & COPD including chronic bronchitis & pulmonary emphysema.
Dosage/Direction for Use
Acute asthma attack Adult & childn >6 yr Initially 2 inhalations. Patient who requires additional symptomatic control 2 more inhalations after 5 min away from 1st inhalation. Intermittent & long-term treatment Adults & childn >6 yr 1-2 inhalations on each asthma attack. Max: 8 inhalations daily.
Contraindications
Hypersensitivity to ipratropium Br, fenoterol HBr or atropine-like substances. Hypertrophic obstructive cardiomyopathy & tachycardia.
Special Precautions
Immediate hypersensitivity reactions. Acute bronchospasm. Stop treatment if paradoxical bronchospasm occurs. Patients w/ narrow-angle glaucoma, prostatic hyperplasia, or bladder-neck obstruction, particularly if receiving an anticholinergic by another route; underlying severe heart disease including ischemic heart disease, arrhythmia or severe heart failure; DM, recent MI, severe organic heart or vascular disorders, hyperthyroidism, pheochromocytoma. Ocular effects; not to be sprayed into the eyes. Hypokalemia. Bronchial obstruction deterioration in prolonged use. Reevaluation of treatment regimen if patient needs more doses than usual. Concomitant administration w/ other sympathomimetic bronchodilators. Transient ventricular premature contractions, atrial arrhythmia, inverted T waves, junctional rhythm, & prolongation of QT interval, arrhythmias that may result from hypoxia or hypokalemia; increased blood glucose conc possibly due to glycogenolysis; & decreased serum K conc possibly through intracellular shunting w/ frequent use of higher doses or an overdose. May affect ability to drive & use machines. Pregnancy (especially during 1st trimester) & lactation. Childn <6 yr.
Adverse Reactions
Cough, dry mouth, headache, tremor, pharyngitis, nausea, dizziness, dysphonia, tachycardia, palpitations, vomiting, increased systolic BP & nervousness.
Drug Interactions
May enhance pharmacological effect & adverse reactions w/ other β-agonist, anticholinergics & xanthine derivatives. Reduction in bronchodilation may occur w/ β-adrenergic blocking agents eg, ophth & systemic β-adrenergic blocking agents. Increased β-agonist induced hypokalemia w/ xanthine derivatives, corticosteroids & diuretics especially in patients w/ severe asthma. Increased susceptibility to arrhythmias due to hypokalemic result w/ digitalis glycoside eg, digoxin. Potentiated action of fenoterol on vascular system w/ MAOIs or TCAs, or w/in 2 wk of discontinuing these agents. May increase CV adverse effects w/ halogenated hydrocarbon anesth eg, halothane, trichloroethylene & enflurane.
