Tacrolimus
Indications/Uses
Prophylaxis for organ rejection in patients receiving allogenic kidney or liver transplant, used along w/ adrenal corticosteroids.
Dosage/Direction for Use
Adult patients in renal transplantation Recommended initial oral dose: 0.2 mg/kg/day in 2 divided doses 12 hrly. Whole blood conc during mth 1-3 should be maintained between 7-20 ng/mL & 5-15 ng/mL during the 4th mth & onwards. 1st dose should be administered after the renal function has recovered (eg, serum creatinine <4 mg/dL). Patients in liver transplantation 0.1-0.15 mg/kg/day administered in 2 divided doses 12 hrly. Most patients are stable when trough whole blood conc are maintained between 5-20 ng/mL.
Administration
Should be taken on an empty stomach: Take at least 1 hr before or 2-3 hr after meals.
Contraindications
Special Precautions
Increased susceptibility to infection & possible development of lymphoma may result from immunosuppression. Increased risk of developing lymphomas & other malignancies particularly of the skin. Oversuppression of the immune system w/ increase susceptibility to infection. Caution in treatment w/ combined immunosuppressives. HTN. Pregnancy & lactation. Childn.
Adverse Reactions
Tremor, headache, diarrhea, hypertension, nausea & renal dysfunction.
Drug Interactions
Potential additive or synergistic renal function impairment w/ aminoglycosides, amphotericin B and cisplatin. Additive/synergistic nephrotoxicity w/ cyclosporine. Increased Seegraf levels w/ diltiazem, nicardipine, nifedipine, verapamil, clotrimazole, fluconazole, itraconazole, ketoconazole, clarithromycin, erythromycin, troleandomycin, cisapride, metoclopramide, bromocriptine, cimetidine, cyclosporine, danazol, ethinyl estradiol, methylprednisolone, omeprazole, protease inhibitors, nefazodone. Reduced Seegraf levels w/ carbamazepine, phenobarb, phenytoin, rifabutin, rifampin & St. John's wort.
