Piracetam
Indications/Uses
Oral: Patients suffering from myoclonus of cortical origin, irrespective of aetiology; in combination w/ other anti-myoclonic therapies. IV: Cerebrocortical insufficiency disorders or states eg, trauma or surgery, alcoholism, vertigo, cerebrovascular accidents, learning disability, dyslexia and behavioral disorders in children. Adjunct in the treatment of myoclonus seizures & in senile dementia. Protects the cerebral cortex against hypoxia. Recurrent or chronic vertigo. Contract myoclonus.
Dosage/Direction for Use
Oral Adult Usual dose: 800 mg tds. Reduce to 400 mg tds. Max: 4.8 g/day. IV Total daily dose: 30-160 mg/kg/day, bd. May be given in 3 or 4 divided doses. Severe symptoms 12 g daily as an IV infusion. Sickle-cell anemia 160 mg/kg/day in 4 divided doses. Adult Stroke (for cerebrovascular accident) 12 g IV in 20 min followed by 12 g/day including the 1st day for 4 wk, then 4.8 g/day for at least 12 wk. Childn Stroke (for cerebrovascular accident) 30-50 mg/kg/day. Elderly Long-term therapy for psycho-organic syndrome 1.2-2.4 g daily. Loading dose: 4.8 g/day during the initial wk of treatment.
Contraindications
Severe renal insufficiency. IV: Pregnancy.
Special Precautions
Monitoring of renal function. Oral: Pregnancy. IV: Patients w/ psychiatric anxiety.
Adverse Reactions
Oral: Nervousness, agitation, irritability, anxiety & sleep disturbances. Nausea, vomiting, diarrhoea & stomachache. Vertigo, headaches, trembling & sexual stimulation. IV: Psychiatric hyperkinesia, sleep disturbance, insomnia, nervousness, aggression, depressive emotional inversion, anxiety, irritability, agitation, drowsiness, decrease of seizure threshold, hyperactivity in children, dizziness, headache, tremor, fatigue. Nausea, vomiting, abdominal discomfort, diarrhoea, gastralgia. Elevation or decrease in BP. Libido, wt gain, allergic reaction, redness, rash, hypersensitivity.
Drug Interactions
Oral: Confusion, irritability & sleep disturbances w/ thyroid extr (T3 and T4). IV: Enhanced CNS stimulation effects w/ CNS stimulants; may cause atypical interaction w/ psychiatric drugs; may enhance effect & cause confusion, hypersensitivity, sleep disturbance w/ thyroid hormone in patients w/ hypothyroidism; prolonged prothrombin time w/ warfarin.
