Indications and Dosage
Oral
Hypertension
Adult: 20 mg daily as a single dose or in 2 divided doses, may be increased to 20 mg bid as necessary to control resistant HTN.
Elderly: Initially, 10 mg daily.
Hepatic Impairment
Initially, 5-10 mg once daily.
Administration
Should be taken with food.
Contraindications
Unstable angina pectoris, use w/in 1 mth of MI, cardiogenic shock.
Special Precautions
Patients w/ severe aortic stenosis, poor cardiac reserve, heart failure. Hepatic impairment. Elderly. Pregnancy and lactation.
Adverse Reactions
Headache, fatigue, flushing, palpitations, dizziness, peripheral oedema, polyuria, elevated LFTs.
Drug Interactions
Increased serum levels w/ cimetidine. Decreased serum levels w/ bile acids (e.g. ursodeoxycholic acid).
Food Interaction
May increase serum concentration w/ grapefruit juice.
Action
Description: Nitrendipine relaxes coronary vascular smooth muscles by inhibiting Ca ions. It lowers BP primarily by arteriolar dilation.
Pharmacokinetics:
Absorption: Well absorbed from the GI tract. Absolute bioavailability: 10-30%. Time to peak plasma concentration: 1-3 hr.
Distribution: Plasma protein binding: Approx 98%.
Metabolism: Metabolised in the liver. Undergoes extensive first-pass metabolism.
Excretion: Via urine and faeces (as inactive metabolites, <0.1% as unchanged drug). Terminal elimination half-life: Approx 10-22 hr.
Storage
Protect from light.
