Ondansetron HCl
Indications/Uses
Management of cytotoxic chemotherapy- & radiotherapy-induced nausea & vomiting. Prevention & treatment of post-op nausea & vomiting.
Dosage/Direction for Use
Adult Emetogenic chemotherapy & radiotherapy 8 mg slow IV or IM inj immediately before treatment followed by 8 mg orally 12 hrly. To protect against delayed or prolonged emesis after 1st 24 hr, continue w/ oral or rectal dose up to 5 days after a course of treatment. High emetogenic chemotherapy eg, high-dose cisplatin Dose schedules: 8 mg slow IV or IM inj single-dose immediately before chemotherapy. 8 mg slow IM inj immediately before chemotherapy followed by 2 further 8-mg dose IV or IM, 2-4 hr apart or by 1 mg/hr constant infusion up to 24 hr. 32 mg single-dose diluted in saline or other compatible infusion fluid 50-100 mL, infused not less than 15 min immediately before chemotherapy. May be enhanced w/ dexamethasone Na phosphate 20 mg single IV dose prior to chemotherapy. Continue oral or rectal treatment up to 5 days after a course of treatment to protect against delayed or prolonged emesis. Prevention of post-op nausea & vomiting 4 mg single IM or slow IV inj dose. Childn Emetogenic chemotherapy & radiotherapy 5 mg/m2 as single IV dose immediately before chemotherapy, followed by 4 mg orally 12 hrly. Continue 4 mg orally bd for up to 5 days after a course of treatment. ≥2 yr Prevention of post-op nausea & vomiting 0.1 mg/kg inj. Max: 4 mg. Moderate or severe hepatic impairment Should not exceed 8 mg total daily dose.
Contraindications
Special Precautions
Hypersensitivity to other 5HT3 receptor antagonists. Transient ECG changes including QT interval prolongation. Patients w/ cardiac rhythm or conduction disturbances; w/ anti-arrhythmic agents or β-adrenergic blockers; w/ significant electrolyte disturbances. Increases large bowel transit time. Patients w/ signs of subacute intestinal obstruction. Pregnancy & lactation.
Adverse Reactions
Headache; sensation of warmth or flushing; constipation; local IV site reactions.
Drug Interactions
Decreased conc levels w/ potent CYP3A4 inducer (ie, phenytoin, carbamazepine, rifampicin, tramadol).
