Indications and Dosage

Topical/Cutaneous
Corticosteroid-responsive dermatoses

Adult: As 0.05% cream, oint, or lotion: Apply sparingly onto the affected areas 2-4 times daily until optimal response is achieved. Max duration of therapy: 8 wk.
Child: ≥ 2 yr Same as adult dose.

Topical/Cutaneous
Atopic dermatitis

Adult: As 0.05% foam or gel: Apply to affected area(s) bid. Max duration of therapy: 4 wk.
Child: ≥3 mth Same as adult dose.

Contraindications

Viral skin infections (e.g. vaccinia, herpes simplex), TB, acne and rosacea; ulcerated lesions.

Special Precautions

Childn. Pregnancy and lactation.

Adverse Reactions

Pruritus, pain, folliculitis, rash, peripheral oedema, pustular rash, sweating, erythema, irritation, burning, itching, dryness, hypertrichosis, acneiform eruptions, hypopigmentation, perioral dermatitis, allergic contact dermatitis, skin maceration/atrophy, secondary infection, striae and miliaria. Adrenal suppression, manifestations of Cushing syndrome, hyperglycaemia or glycosuria, facial swelling, withdrawal syndrome, growth retardation in childn (systemic).

Patient Counseling Information

Avoid use w/ occlusive dressing. Desonide foam is flammable, do not smoke during or immediately after application.

Monitoring Parameters

Monitor for hypothalamic-pituitary-adrenal (HPA) axis suppression thru ACTH stimulation test, plasma cortisol test and urinary free cortisol test; signs and symptoms of bacterial or fungal infection.

Drug Interactions

Enhanced effect w/ other topical corticosteroids.

Action

Description: Desonide is a synthetic corticosteroid that induces phospholipase A₂ inhibitory proteins (lipocortins) and sequentially inhibits release of arachidonic acid, hence depresses the formation, release, and activity of chemical inflammatory mediators.
Pharmacokinetics:
Absorption: Absorbed through normal skin; increased percutaneous absorption in traumatised or inflamed skin.
Metabolism: Metabolised in the liver.
Excretion: Mainly via urine; bile.
