Indications/Uses
Dosage/Direction for Use
Adult & adolescent >12 yr 50-100 mg slow IV inj over 2-3 min 4-6 hrly. Post-op pain 100 mg as initial bolus followed by 50 mg every 10-20 min after 60 min of initial bolus, up to total dose: 250 mg then 50-100 mg 4-6 hrly up to total daily dose: 400 mg.
Contraindications
Hypersensitivity. Uncontrolled epilepsy. Narcotic w/drawal treatment. Acute alcohol, hypnotics, analgesics, opioids or psychotropic medications intoxication. Concomitant or use of MAOIs w/in last 14 days. Uncontrolled epilepsy. Adolescents 12-18 yr w/ obesity, obstructive sleep apnea or severe lung disease. Childn <18 yr after tonsils &/or adenoids removal.
Special Precautions
Sensitivity to opiates. Not suitable as substitute in opioid-dependent patients. Epileptic or patients susceptible to seizures. Patients w/ head injury, shock, reduced level of consciousness of uncertain origin, resp centre or function disorders, increased intracranial pressure; resp depression; decreased resp reserve, hypoxia, hypercapnia or pre-existing resp depressions. Tolerance, drug abuse, psychological & physical dependence. CYP2D6 ultra-rapid metabolizers. Avoid abrupt w/drawal. Concomitant use w/ CNS depressants; medications lowering seizure threshold; sedatives eg, benzodiazepines or related drugs. May affect ability to drive & use machines. Renal & hepatic insufficiency; on dialysis. Not recommended during pregnancy. Not to be used during lactation. Post-op pain relief in childn. Not recommended in childn w/ compromised resp function eg, neuromuscular disorders, severe cardiac or resp conditions, upper resp or lung infections, multiple trauma or extensive surgical procedures & childn <12 yr. Elderly >75 yr.
Adverse Reactions
Dizziness; nausea. Headache, somnolence; vomiting, constipation, dry mouth; sweating; fatigue.
Drug Interactions
Not to be used w/ MAOIs. Reduced analgesic effect & duration of action w/ carbamazepine. Increased risk of convulsions w/ SSRIs, SNRIs, TCAs, antipsychotics & other seizure threshold-lowering medications eg, bupropion, mirtazapine, tetrahydrocannabinol. Serotonin syndrome w/ SSRIs, SNRIs, MAOIs, TCAs & mirtazapine. Increased INR w/ coumarin derivatives eg, warfarin. Inhibited metabolism w/ CYP3A4 inhibitors eg, ketoconazole & erythromycin. Increased requirement w/ pre- & post-op use of ondansetron in patients w/ post-op pain. Potentiated CNS effects w/ centrally-depressant drugs eg, alcohol. Increased risk of sedation, resp depression, coma & death w/ benzodiazepines or related drugs. Precipitation w/ diazepam, diclofenac Na, indomethacin, midazolam, piroxicam.
