Indications/Uses
Anxiety, mixed anxiety- & neurotic or reactive depression, panic-related disorder.
Dosage/Direction for Use
Anxiety Initially 0.75-1.5 mg daily in divided doses. Usual dose range: 0.5-4 mg daily in divided doses. Depression Initially 1.5 mg daily in divided doses. Usual dose range: 1.5-4.5 mg daily in divided doses. Panic disorders Initially 0.5-1 mg at bedtime or 0.5 mg tds. Usual dose range: Adjusted according to patient's response in increments not >1 mg daily every 3-4 days. Elderly Initially 0.5-0.75 mg daily in divided doses. Usual dose range: 0.5-0.75 mg daily in divided doses, may be gradually increased if needed.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to benzodiazepines. Myasthenia gravis, severe resp insufficiency, sleep apnoea syndrome. Severe hepatic insufficiency.
Special Precautions
Anaphylaxis & angioedema. Discontinue use if psychiatric & paradoxical reactions occur. Not recommended as primary treatment of psychotic illness; not to be used alone for treatment of depression or anxiety-associated depression. Severely depressed or suicidal patients. Complex sleep-related behaviors; chronic resp insufficiency. History of alcohol or drug abuse. May develop tolerance, dependence, rebound insomnia & anxiety; induce anterograde amnesia. Avoid abrupt w/drawal. Not for long-term use. Combination w/ CNS depressants, alcohol. May affect ability to drive & use machines. Not to be used in severe hepatic insufficiency. Women of childbearing potential. Avoid use during pregnancy. Not be used during lactation. Not recommended in childn <18 yr. Elderly.
Adverse Reactions
Sedation/drowsiness, lightheadedness, numbed emotions, reduced alertness, confusion, fatigue, headache, dizziness, memory impairment, muscle weakness, ataxia, double or blurred vision, insomnia, nervousness/anxiety, tremor, wt change. GI disturbances, skin reactions; dystonia, anorexia, slurred speech, jaundice, sexual dysfunction/changes in libido, menstrual irregularities, incontinence, urinary retention, abnormal liver function, hyperprolactinaemia.
Drug Interactions
Enhanced sedative effect w/ alcohol. Enhanced central depressive effect w/ antipsychotics (neuroleptics), hypnotics, anxiolytics/sedatives, antidepressants, narcotic analgesics, antiepileptics, anaesth & sedative antihistamines. Enhanced euphoria w/ narcotic analgesics. Not recommended w/ azole-type antifungals eg, ketoconazole, itraconazole. Concomitant use w/ nefazodone, fluvoxamine, cimetidine, fluoxetine, propoxyphene, OCs, diltiazem, macrolide antibiotics eg, erythromycin, troleandomycin. Impaired clearance, prolonged t½ & enhanced clinical effects w/ ritonavir.
