Indications/Uses
Serious or severe staph infections in penicillin-allergic patients for infections resistant to other antimicrobial drugs.

Dosage/Direction for Use
Slow IV infusion Adult 500 mg 6 hrly or 1 g 12 hrly. Childn 44 mg/kg daily in divided dose (6 hrly). Neonate Initially 15 mg/kg followed by 10 mg/kg 12 hrly in the 1st wk of life & 8 hrly up to 1 mth. Renal impairment Initially 15 mg/kg. CrCl 2 mL/min/kg 30.9 mg/kg daily, 1.5 mL/min/kg 23.2 mg/kg daily, 1 mL/min/kg 15.4 mg/kg daily, 0.5 mL/min/kg 7.7 mg/kg daily, 0.2 mL/min/kg 3.1 mg/kg daily. Anephric patient Loading dose: 15 mg/kg w/ subsequent dosage of 1.9 mg/kg daily. Anuria 1 g every 7-10 days. Haemodialysis patient 1 g every 7 days.

Special Precautions
Avoid rapid bolus administration. Prolonged therapy. Patients w/ previous hearing loss. Perform periodic blood, urine, liver & renal function tests. Anaesth-induced myocardial depression Avoid concurrent or sequential use of other nephrotoxic drugs. Renal impairment. Pregnancy & lactation. Elderly.

Adverse Reactions
Reversible deafness; hypotension, palpitations, substernal pressure, phlebitis; inj site reactions, generalised flushing, erythematous macular rash, bullous eruption disorders; overgrowth of nonsusceptible organisms; reversible neutropenia, eosinophilia; histamine release w/ chills, nausea, urticaria, fever & rigors, hypersensitivity reactions; pain, chest & back muscle spasms.
Drug Interactions
Erythema, histamine ie, flushing & anaphylactoid reactions w/ anaesth. Careful monitoring w/ concurrent administration of neurotoxic or nephrotoxic drugs eg, amphotericin B, streptomycin, neomycin, gentamicin, kanamycin, amikacin, tobramycin, viomycin, bacitracin, polymyxin B, colistin, cisplatin. Aggravated toxicity w/ diuretics eg, ethacrynic acid, frusemide. In vitro binding by cholestyramine.
