Indications/Uses
Prophylaxis & treatment of venous thrombosis & pulmonary embolism. MI & arterial embolism. Prevention of clotting in arterial & heart surgery; cerebral thrombosis. Anticoagulant in blood transfusion, extracorporal circulation, dialysis procedures & for lab purposes.
Dosage/Direction for Use
IV 5,000-10,000 IU every 4 hr or 500 IU/kg daily as continuous infusion. SC Initially 250 IU/kg every 12 hr. Patient undergoing major elective surgery 5,000 IU 2 hr pre-op & then every 8-12 hr post-op for 10-14 days. Following MI 5,000 IU bd for 10 days. Other patient 5,000 IU every 8-12 hr. Prophylaxis during pregnancy 5,000 IU every 12 hr in early pregnancy, may be increased to 10,000 IU every 12 hr in last trimester.
Contraindications
Hypersensitivity. Not to be used in presence of actual or potential haemorrhagic states eg, haemophilia, ascorbic acid deficiency or increased capillary fragility; threatened abortion; immediate postpartum period, subacute bacterial endocarditis; severe HTN; gastric or duodenal ulcers; spinal or major surgery. Advanced renal or hepatic disease.
Special Precautions
Hypersensitivity to LMWH. Discontinue use if thrombocytopenia develops. DM, preexisting metabolic acidosis, raised plasma K. Use of peridural or spinal catheter anesth; traumatic or repeated puncture. Monitor plasma K prior to & regularly thereafter. Measure platelet counts during therapy. Concomitant use w/ K-sparing drugs; NSAIDs, platelet inhibitors, anticoagulants. Chronic renal failure. Renal & hepatic impairment. Pregnancy. Avoid use in childn <2 yr. Not to be used in neonates.
Adverse Reactions
Haemorrhage; transient alopecia, diarrhea; osteoporosis w/ spontaneous bone fracture.
Drug Interactions
Increased risk of haemorrhage w/ drugs affecting platelet function eg, aspirin, other NSAIDs, dextran, dipyridamole, thrombolytic agents eg, streptokinase, urokinase. Incompatible w/ hydrocortisone, hyaluronidase, hydroxyzine, antihistamines, narcotic analgesics, phenothiazines, antibiotics.
