Clindamycin (Cap: HCl; Inj: Phosphate)
Indications/Uses
Staph, strep (except Strep faecalis), pneumococci, susceptible Chlamydia trachomatis strains & anaerobic bacterial infections.
Dosage/Direction for Use
Cap Adult Serious infections 150-300 mg 6 hrly. More severe infections 300-450 mg 6 hrly. Childn Serious infections 8-16 mg/kg daily in 3-4 divided doses. More severe infections 16-20 mg/kg daily in 3-4 divided doses. Pneumocystis jiroveci pneumonia in patient w/ AIDS 300-450 mg orally every 6 hr + primaquine 15-30 mg once daily for 21 days. Inj Adult Intra-abdominal area, female pelvis & other complicated or serious infections 2,400-2,700 mg in 2, 3 or 4 equal doses. Less complicated infections 1,200-1,800 mg daily in 3 or 4 equal doses. Max: 4,800 mg daily or single dose 600 mg IM. Childn >1 mth 20-40 mg/kg daily in 3 or 4 equal doses. Neonate <1 mth 15-20 mg/kg daily in 3 or 4 equal doses. Small premature infant 15 mg/kg daily in 3 or 4 equal doses. β-hemolytic Streptococcal infections Continue treatment for at least 10 days. Inpatient treatment of pelvic inflammatory disease 900 mg IV every 8 hr + gentamicin 2 mg/kg followed by 1.5 mg/kg every 8 hr for at least 4 days & at least 48 hr after improvement. Toxoplasmic encephalitis in patient w/ AIDS 600-1,200 mg every 6 hr for 2 wk followed by 300-600 mg orally every 6 hr. Total duration: 8-10 wk. Pneumocystis jiroveci pneumonia in patient w/ AIDS 600-900 mg every 6 hr or 900 mg every 8 hr.
Administration
Should be taken with food: Take w/ a full glass of water.
Contraindications
Hypersensitivity to clindamycin or lincomycin. Newborn.
Special Precautions
History of GI disease, colitis, diarrhoeal states. Prolonged use. Renal & hepatic impairment. Pregnancy & lactation. Inj: Not to be used as undiluted IV bolus inj. Not to be used in nonbacterial infections & meningitis treatment. Avoid use in childn <2 yr. Not to be used in neonates.
Adverse Reactions
Diarrhoea, pseudomembranous colitis (discontinue immediately), nausea, vomiting, abdominal cramps, dysgeusia, rash, urticaria, transient leucopenia, eosinophilia, jaundice, agranulocytosis, thrombocytopenia, erythema multiforme. Inj: Local irritation, pain, abscess formation.
Drug Interactions
Kaolin-containing anti-diarrhoeal prep. Antagonism w/ erythromycin. Inj: Enhanced action w/ neuromuscular blockers.
