Alprazolam
Indications/Uses
Short-term treatment of moderate or severe anxiety or w/ depression.
Dosage/Direction for Use
Anxiety 0.25-0.5 mg tds, may be increased to 3 mg daily. Elderly or debilitated patient 0.25 mg bd-tds. Duration of treatment: Not >8-12 wk.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to benzodiapines. Myasthenia gravis, severe resp & hepatic insufficiency, sleep apnoea syndrome.
Special Precautions
Not to be used for short-term mild anxiety; monotherapy for anxiety w/ depression. Not recommended for primary treatment of psychotic illness. Anaphylaxis & angioedema. Tolerance & dependence. Complex sleep-related behaviors. Amnesia; psychiatric & paradoxical reactions; chronic resp insufficiency; severely depressed or suicidal patients. History of alcohol & drug abuse. Avoid abrupt discontinuation. Concomitant use w/ opioids. May affect ability to drive & use machines. Severe hepatic insufficiency. Renal function. Pregnancy. Not to be given during lactation. Not recommended in childn. Elderly.
Adverse Reactions
Sedation/drowsiness, lightheadedness, numbed emotions, reduced alertness, confusion, fatigue, headache, dizziness, muscle weakness, ataxia, double or blurred vision, insomnia, nervousness/anxiety, tremor, wt change, GI disturbances, changes in libido, skin reactions.
Drug Interactions
Enhanced sedative effects w/ alcohol. Enhanced central depressive effect w/ antipsychotics, hypnotics, anxiolytics/sedatives, antidepressants, narcotic analgesics, antiepileptics, anaesth, sedative antihistamines. Increased conc w/ ketoconazole, itraconazole, other azole antifungals. Impaired clearance w/ HIV PIs eg, ritonavir. Increased risk of resp depression, profound sedation, coma & death w/ opioids. Nefazodone, fluvoxamine, cimetidine; fluoxetine, propoxyphene, OCs, sertraline, diltiazem, macrolide antibiotics eg, erythromycin, troleandomycin.
