Cilostazol
Indications/Uses
Improvement of maximal & pain-free walking distances in patients w/ intermittent claudication, who do not have rest pain & evidence of peripheral tissue necrosis. Prevention of cerebral infarction recurrence (excluding cardiogenic cerebral embolism).
Dosage/Direction for Use
100 mg bd, may be given up to 12 wk.
Administration
Should be taken on an empty stomach: Take at least 30 min before or 2 hr after meals.
Contraindications
Hypersensitivity. CHF. Patients w/ haemostatic disorders or active pathologic bleeding eg, peptic ulcer & intracranial bleeding.
Special Precautions
Patients at risk of bleeding from surgery or pathologic processes; thrombocytopenia. Patients w/ unstable angina pectoris, MI, or coronary intervention w/in last 6 mth; history of tachyarrhythmia. Periodically monitor WBC &/or platelet counts in long-term use. Concomitant use w/ antiplatelet or anticoagulants. May affect ability to drive & operate machinery. Severe hepatic & renal impairment. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Headache, dizziness; palpitation, tachycardia; diarrhea, dyspepsia, nausea, abdominal pain; peripheral edema.
Drug Interactions
Increased absorption w/ high-fat meal. Increased AUC w/ clopidogrel. Increased plasma levels w/ aspirin. Increased blood levels w/ CYP3A4 inhibitors eg, itraconazole, erythromycin, diltiazem, ketoconazole & CYP2C19 inhibitors eg, omeprazole.
