Timolol maleate
Indications/Uses
Elevated IOP in patients w/ ocular HTN or open-angle glaucoma.
Dosage/Direction for Use
Instill 1 drop in affected eye(s) bd. Transfer from other topical β-blocker Discontinue β-blocker after full day of therapy & start treatment w/ 1 drop bd the next day. Transfer from single antiglaucoma agent other than topical β-blocker Continue agent & add 1 drop to each affected eye bd. Discontinue previous agent the following day & continue Optimolol.
Contraindications
Hypersensitivity. Reactive airway disease including bronchial asthma or history of bronchial asthma, severe COPD; sinus bradycardia, sick sinus syndrome, SA block, 2nd- or 3rd-degree AV block, overt cardiac failure or cardiogenic shock.
Special Precautions
History of atopy or severe anaphylactic reaction to variety of allergens. Adequately control cardiac failure prior to therapy. History of CV diseases, 1st-degree heart block; severe peripheral circulatory disturbance/disorders eg, severe forms of Raynaud's disease or Raynaud's syndrome; mild/moderate COPD; spontaneous hypoglycemia or labile diabetes; corneal diseases; choroidal detachment. Patients w/ dry eyes, compromised corneas, angle-closure glaucoma. Muscle weakness w/ certain myasthenic symptoms eg, diplopia, ptosis, generalised weakness. Intercurrent ocular condition eg, trauma, ocular surgery or infection; bacterial keratitis. May mask signs of acute hypoglycemia, hyperthyroidism. May cause eye irritation due to benzalkonium Cl. Remove contact lens prior to application & reinsert after 15 min. Allow 10-min interval if >1 topical ophth drug is used. Avoid contact w/ container tip. Prolonged use. Not recommended w/ another topical β-blocker. Concomitant use w/ surgical anaesth. May affect ability to drive & use machines. Not to be used during pregnancy. Lactation. Neonates, infants & younger childn.
Adverse Reactions
Ocular irritation, conjunctivitis, blepharitis, keratitis, dry eyes, decreased corneal sensitivity, blurred vision, corneal erosion, visual disturbances, diplopia, ptosis, choroidal detachment; tinnitus; bradycardia, chest pain, arrhythmia, heart block, CHF, palpitations, cardiac arrest & failure, oedema; claudication, hypotension, Raynaud's phenomenon, cold hands & feet; bronchospasm, resp failure, dyspnoea, cough; asthenia, fatigue; alopecia, psoriasiform rash or psoriasis exacerbation, skin rash; SLE, pruritus; depression, insomnia, nightmares, memory loss, hallucination; syncope, CVA, cerebral ischemia, headache, dizziness, increased signs & symptoms of myasthenia gravis, paraesthesia; nausea, diarrhoea, dyspepsia, dry mouth, dysgeusia, abdominal pain, vomiting; decreased libido, Peyronie's disease, sexual dysfunction; hypoglycaemia; myalgia.
Drug Interactions
Potentiated additive effects w/ oral Ca-channel blockers, β-adrenergic blockers, antiarrhythmics (eg, amiodarone), digitalis glycosides, rauwolfia alkaloids, parasympathomimetics, guanethidine; catecholamine-depleting drugs eg, reserpine. Mydriasis w/ epinephrine (adrenaline). Potentiated systemic β-blockade w/ CYP2D6 inhibitors eg, quinidine, fluoxetine, paroxetine. Exacerbated rebound HTN following w/drawal of clonidine. Prolonged AV conduction time w/ digitalis & diltiazem/verapamil. Increased hypoglycaemic effect of antidiabetics.
