Chlorambucil
Indications/Uses
Hodgkin's disease, certain forms of non-Hodgkin's lymphoma, chronic lymphocytic leukaemia, Waldenstrom's macroglobulinaemia.
Dosage/Direction for Use
Hodgkin's disease 0.2 mg/kg daily for 4-8 wk. Non-Hodgkin's lymphoma 0.1-0.2 mg/kg daily for 4-8 wk. Maintenance: Reduced daily dosage or intermittent courses of treatment. Chronic lymphocytic leukaemia Initially 0.15 mg/kg daily until total leukocyte count falls to 10,000/microliter. Resume treatment 4 wk after at dose: 0.1 mg/kg daily. Waldenstrom's macroglobulinaemia Starting dose: 6-12 mg daily until leuopenia occurs followed by 2-8 mg daily.
Administration
Should be taken on an empty stomach: Take at least 1 hr before or 3 hr after meals. Swallow whole, do not chew/crush. Ensure adequate hydration.
Contraindications
Hypersensitivity. 1st trimester of pregnancy & lactation.
Special Precautions
Not to be used long-term in patients w/ potential to have autologous stem cell transplantation. Not be given to patients who have recently undergone RT or received other cytotoxic agents. History of seizure disorders, patients on high pulse dosing regimens; lymphocytic bone marrow infiltration or hypoplastic bone marrow; chromatid or chromosome damage; acute secondary haematologic malignancies; significantly increased incidence of acute leukaemia. Monitor renal function, signs & symptoms of hepatic toxicity. Closely monitor blood counts during treatment. Not recommended w/ live organism vaccine in immunocompromised individuals. Renal & severe hepatic impairment. May impair fertility in women & men. Avoid use during pregnancy. Not to be used during lactation. Childn w/ nephrotic syndrome.
Adverse Reactions
Leukopenia, neutropenia, thrombocytopenia, pancytopenia, bone marrow suppression. Acute secondary haematologic malignancies especially leukaemia & myelodysplastic syndrome (long-term); anaemia; convulsions in childn w/ nephrotic syndrome; GI disorders eg, nausea, vomiting, diarrhoea, mouth ulceration.
Drug Interactions
Not recommended w/ live organism vaccines. Increased cytotoxicity w/ purine nucleoside analogues eg, fludarabine, pentostatin, cladribine.
