Indications/Uses

Severe sleep disorders in cases of occasional & transient insomnia.

Dosage/Direction for Use

Adult 10 mg daily taken before bedtime. Elderly w/ hepatic insufficiency 5 mg daily. Max: 10 mg daily. Duration: Occasional insomnia eg, during trip 2-5 days. Transient insomnia eg, during troubled period 2-3 wk.

Overdosage

Administration

May be taken with or without food.

Contraindications

Hypersensitivity. Obstructive sleep apnoea, myasthenia gravis, acute &/or severe resp depression. Severe hepatic insufficiency. Childn or patients w/ psychotic illness.

Special Precautions

Anaphylaxis & angioedema. Discontinue use if psychiatric & paradoxical reactions or sleep-related behaviours (eg, sleep driving) occur. Depression; compromised resp function; history of drug or alcohol abuse & psychiatric disorders; rebound insomnia. May develop tolerance & dependence; induce anterograde amnesia. Avoid abrupt w/drawal. Combination w/ alcohol & other CNS depressants. May affect ability to drive & use machines. Hepatic insufficiency. Women of childbearing potential. Avoid use during pregnancy. Not recommended during lactation. Childn <18 yr. Elderly.

Adverse Reactions

Hallucination, agitation, nightmare; somnolence, headache, dizziness, exacerbated insomnia, anterograde amnesia; diarrhea; fatigue.

Drug Interactions

Enhanced sedative effects w/ alcohol. Enhanced central depressive effects w/ antipsychotics (neuroleptics), hypnotics, anxiolytics/sedatives, antidepressants, narcotic analgesics, antiepileptics, anaesth & sedative antihistamines. Increased drowsiness w/ sertraline. Enhanced euphoria w/ narcotic analgesics. Enhanced activity of benzodiazepines & benzodiazepine-like agents w/ CYP450 inhibitors. Decreased pharmacodynamic effect w/ rifampicin. Prolonged elimination t_½, increased total AUC & decreased apparent oral clearance w/ ketoconazole.
