Indications/Uses
MRSA & VRE of the following infection caused by gm +ve bacteria: Nosocomial pneumonia; complicated skin & soft tissue infections including diabetic foot infections w/o concomitant osteomyelitis caused by S. aureus, S. pyogenes or S. agalactiae.
Dosage/Direction for Use
Adult Nosocomial pneumonia, complicated skin & soft tissue infections 600 mg IV or PO 12 hrly for 10-14 days. Paed (6-11 yr) 10 mg/kg IV or PO 8 hrly for 10-14 days. Adult Infection due to MRSA 600 mg 12 hrly.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Patients taking MAOIs & sympathomimetic agents, patients w/ carcinoid syndrome or patient taking serotonin reuptake inhibitors, TCAs, triptans, meperidine or buspirone; patients w/ uncontrolled HTN, pheochromocytoma, thyrotoxicosis.
Special Precautions
Monitor platelet counts for patients w/ preexisting myelosuppression (including anemia, leucopenia, pancytopenia & thrombocytopenia), increased risk of bleeding, receiving concomitant medications that may decrease platelet count of linezolid for >2 wk. Visual function should be monitored (extended period). Lactic acidosis, serotonin syndrome. Pregnancy & lactation.
Adverse Reactions
Diarrhea, headache, nausea. Less common: Oral moniliasis, HTN, dyspepsia, localized abdominal pain, pruritus, tongue discoloration, thrombocytopenia.
Drug Interactions
Adrenergic & serotonergic agents, food or beverage w/ high tyramine content (eg, cheese). Rifampicin, gentamicin, aztreonam.
