Indications/Uses
Dosage/Direction for Use
Surgical anesth Epidural surgery 50-150 mg/10-20 mL of 0.5-0.75% conc moderate to complete motor block; epidural for caesarean section 75-150 mg/15-30 mL of 0.5% conc moderate to complete motor block; peripheral nerve Max: 150 mg/1-40 mL of 0.25-0.5% conc moderate to complete motor block; ophth 37.5-112.5 mg/5-15 mL of 0.75% conc moderate to complete motor block; local infiltration Max: 150 mg/1-60 mL of 0.25% conc. Pain management Labor analgesia (epidural bolus) 25-50 mg/10-20 mL in 0.25% conc minimal to moderate motor block; post-op pain (epidural infusion) 12.5-18.75 mg/hr/10-15 mL/hr or 12.5-18.75 mg/hr/5-7.5 mL/hr in 0.125-0.25% conc minimal to moderate motor block.
Contraindications
Hypersensitivity to levobupivacaine or any local anaesth agent of the amide type. Patients w/ severe hypotension eg, cardiogenic or hypovalaemic shock. Bier block, paracervical block & 0.75% Vopicain use in obstetric procedure. Do not use for IV regional anesth.
Special Precautions
Not recommended for emergency situations. Not for the production of obstetrical paracervical block anaesth. Patients receiving other local anesth or agents structurally related to amide-type local anesth. Do not inject IV regional anesth (Bier block). Patients w/ impaired CV function, severe hepatic disease. Careful & constant monitoring of CV & resp vital signs (adequacy of ventilation) & perform patient's state of consciousness after each local anesth inj. A test dose must be administered initially during epidural anesth & monitor effects before the full dose is given. Monitor respiration & circulation during administration of small doses of local anesth injected into the head & neck. May cause temporary loss of sensation & motor activity in the anesthesized part of the body following correct administration of regional anesth. Pregnancy.
Adverse Reactions
Asthenia, edema, postural hypotension, hypokinesia, involuntary muscle contraction, spasm (generalized), tremor, syncope, arrhythmia, extrasystoles, atrial fibrillation, cardiac arrest, ileus, elevated bilirubin, confusion, apnea, bronchospasm, dyspnea, pulmonary edema, resp insufficiency, increased sweating, skin discoloration.
Drug Interactions
May result to additive effects w/ local anesth or agents structurally related to amide-type local anesth; anti-arrhythmic agents w/ local anesth eg, mexilitine or class III anti-arrhythmic agents.
