Vancomycin HCl
Indications/Uses
Serious or severe infections due to methicillin-resistant Staph endocarditis.
Dosage/Direction for Use
Adult 500 mg 6 hrly or 1 g IV 12 hrly over a period of 60 min. Max: 10 mg/min. Childn 10 mg/kg IV 6 hrly over a period of 60 min. Infant & neonate Initially 15 mg/kg, followed by 10 mg/kg 12 hrly for neonates in 1st wk & 8 hrly thereafter up to 1 mth of therapy.
Contraindications
Hypersensitivity.
Special Precautions
May cause hearing loss. Kidney failure; prolonged use may cause overgrowth of nonsusceptible microorganism. Pregnancy & lactation.
Adverse Reactions
Cold, nausea, trembling, urticaria, anaphylaxis, eosinopilia, neutropenia, SJS, vertigo, dizziness, ototoxicity, tinnitus, TEN, vasculitis, fever, hypotension, nephrotoxicity, skin redness, thrombophlebitis, inj site pain.
Drug Interactions
Aminoglycosides, amphotericin B, aspirin or salicylate, bacitracin, bumetanide, capreomycin, carmustin, cisplatin, cyclosporine, ethacrynic acid, furosemide, paromomycin, polymyxin, cyclizine, meclizine, phenothiazine, trimethobenzamide.
