Doripenem
Indications/Uses
Nosocomial pneumonia (including ventilator-associated pneumonia); complicated intra-abdominal infections.
Dosage/Direction for Use
Nosocomial pneumonia (including ventilator-associated pneumonia) 500 mg 8 hrly infused in 1 or 4 hr. Complicated intra-abdominal infections 500 mg 8 hrly infused in 1 hr. Usual treatment duration: 5-14 days. Patient w/ moderate renal impairment (CrCl 30-50 mL/min) 250 mg 8 hrly. Patient w/ severe renal impairment (CrCl <30 mL/min) 250 mg 12 hrly.
Contraindications
Hypersensitivity to doripenem, any other carbapenem antibacterial agent or any other β-lactam antibacterial agent eg, penicillins or cephalosporins.
Special Precautions
Previous history of hypersensitivity reaction to β-lactam antibiotics. Discontinue use if hypersensitivity occurs. Seizures. Pseudomembranous colitis due to C. difficile. Patients w/ diarrhea during or subsequent to therapy. Monitor patients carefully during therapy for superinfection. Avoid prolonged use. Not to be administered by inhalation as pneumonitis can occur. Pregnancy & lactation. Childn <18 yr.
Adverse Reactions
Headache; diarrhea, nausea. Oral candidiasis, vulvomycotic infection; hypersensitivity reactions; phlebitis; colitis caused by C. difficile; increased hepatic enzyme; pruritus, rash.
Drug Interactions
May reduce serum valproic acid conc. Reduced renal clearance & increased AUC w/ probenecid.
