Ketorolac trometamol
Indications/Uses
Short-term management of moderate post-op pain.
Dosage/Direction for Use
Adult 10 mg every 4-6 hr. Max: 40 mg daily. Elderly & renal impairment Use lowest dose range w/ longer dose interval of 6-8 hr. Max duration of treatment: 7 days.
Administration
Should be taken with food: Take immediately after meals.
Contraindications
Hypersensitivity to ketorolac or other NSAIDs. Active peptic ulcer, cerebrovascular disease, hemorrhagic diatheses including coagulation disorders, hypovolemia; history of asthma, patients under-op w/ high risk of hemorrhage haemostasis, on anticoagulant including low dose heparin (2,500-5,000 U every 12 hr); suspected or confirmed cerebrovascular bleeding, complete or partial nasal polyps syndrome, angioedema or bronchospasm; history of SJS or vesicular bullous rash. Concomitant use w/ oxpentifylline, probenecid, other NSAIDs. Neuraxial (epidural or intrathecal) administration; prophylaxis before major surgery or intra-op when haemostasis is critical due to increased bleeding risk. Moderate to severe renal impairment (serum creatinine >160 μmol/L). Pregnancy, labor, delivery or lactation. Childn <16 yr.
Special Precautions
History of GIT disease, asthma, vol depletion due to blood loss or severe dehydration, coagulation disorders; pre-op medication, support of anesth or obstet analgesia; abrupt discontinuation; cardiac decomposition, HTN. Closely monitor urine output, BUN, serum creatinine & renal function. Elderly >65 yr.
Adverse Reactions
GI disturbances; CNS & musculoskeletal, urinary tract & renal, CV/hematological, resp tract disorders; dermatological & hypersensitivity reactions; bleeding; abnormal liver functions, asthenia, edema, wt gain, hepatic disorder, fever.
Drug Interactions
NSAIDs, oxpentifylline, probenecid, lithium salt; highly protein bound drugs, ACE inhibitors, methotrexate.
