Metoclopramide HCl
Indications/Uses
Symptomatic relief of acute & recurrent diabetic gastroparesis, heartburn & delayed gastric emptying secondary to reflux esophagitis. Prophylaxis of post-op & emetogenic cancer chemotherapy nausea & vomiting. Facilitates intestinal intubation in adult & childn.
Dosage/Direction for Use
Oral Adult 10 mg or 1-2 tsp 3 times daily. Childn & adolescents Max dose: 0.5 mg/kg/day in 3 divided doses. Childn <6 yr Max dose: 0.1 mg/kg/day as a single dose. Post-chemotherapy antiemetic Adult 0.5 mg/kg/day 4 times daily for 6 days, to be given 24 hr after chemotherapy. Inj Treatment of delayed gastric emptying Adult 10 mg IV inj over 1-2 min. Childn 6-14 yr 2.5-5 mg IV inj, ≤6 yr 0.1 mg/kg IV inj. Prevention of vomiting due to chemotherapy 1-2 mg/kg IV inj over >15 min, to be given 30 min before chemotherapy. Prevention of vomiting before surgery 10-20 mg IM inj over 1-2 min, before the end of surgery.
Administration
Should be taken on an empty stomach: Take 30 min before meals.
Contraindications
Epilepsy, GI hemorrhage, mechanical obstruction or perforation, pheochromocytoma.
Special Precautions
Reduce dose in renal impairment as it may increase extrapyramidal symptoms. Procaine & procainamide intolerance. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Headache, restlessness, exhaustion, extrapyramidal effects especially during long-term therapy in childn, constipation, diarrhea & sedation.
Drug Interactions
Effects on gastric motility antagonized by anticholinergic drugs & narcotic analgesics. Increased absorption of acetaminophen. Additive extrapyramidal effects w/ phenothiazines. Increased plasma conc of levodopa.
