Methylprednisolone Na succinate
Indications/Uses
IM or IV use in endocrine, rheumatic & hematologic disorders; collagen & dermatologic, ophth, GI, resp & neoplastic diseases; allergic & edematous states; cerebral edema from tumor-primary or metastatic &/or associated w/ surgical or RT; acute exacerbation of multiple sclerosis; acute spinal cord injury; adjunct to antiTB chemotherapy for the treatment of TB meningitis w/ subarachnoid or impending block; trichinosis w/ neurologic or myocardial involvement.
Dosage/Direction for Use
High dose therapy 30 mg/kg IV over at least 30 min, may be repeated 4-6 hrly for 48 hr. Other indications Initial dosage: 10-40 mg IM/IV. Infant & childn Not <0.5 mg/kg daily.
Contraindications
Hypersensitivity. Systemic fungal infection. Premature infants (vials contain benzyl alcohol). Intrathecal & epidural administration.
Special Precautions
Ocular herpes simplex; psychic derangements; nonspecific ulcerative colitis; active or latent peptic ulcer, renal insufficiency, HTN, osteoporosis, myasthenia gravis; immunization (defer); active or latent TB; unusual stress. May mask signs of infection & new infection may appear. Posterior subcapsular cataracts & glaucoma w/ possible damage to optic nerves may occur. Reduce dose gradually. Pregnancy & lactation.
Adverse Reactions
GI disorders; peptic ulcer w/ possible perforation & hemorrhage; manifestations of latent DM; anaphylactic reactions w/ or w/o circulatory collapse, congestive cardiac failure (in susceptible patients); arrhythmias, bronchospasm. Fluid & electrolyte disturbances.
Drug Interactions
INH, rifampin, erythromycin, ketoconazole, itraconazole, oral anticoagulants, anticonvulsants, anticholinergics, anticholinesterases, antidiabetic drugs, antiemetic, antivirals, aromatase inhibitors, diuretics, live vaccines, Ca channel blocker; cyclosporin, pancuronium, CNS active drugs & salicylates.
