Indications/Uses
As adjunct to diet for reduction of elevated total cholesterol, LDL-C, Apo B, & triglycerides in patients w/ primary hypercholesterolemia, hyperlipidemia, combined homozygous & heterozygous familial hypercholesterolemia if response to diet & other non-pharmacology parameter is not sufficient. Childn 10-17 yr: Adjunct to diet to reduce total cholesterol, LDL-C, Apo B in boys & postmenarchal girls 10-17 yr w/ heterozygous familial hypercholesterolemia if after an adequate trial of diet therapy, the following findings are present: LDL-C remains ≥190 mg/µL or ≥150 mg/µL & there is a positive family history of premature CV disease or ≥2 other CV disease risk factors.
Dosage/Direction for Use
Usual dosage range: 10-80 mg once daily. Initially 10 mg once daily. Combined homozygous & heterozygous familial hypercholesterolemia 10 mg one daily. Homozygous familial hypercholesterolemia 80 mg. Heterozygous familial hypercholesterolemia Childn 10-17 yr Initially 10 mg daily. Max: 20 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Liver disease or persistent elevated serum transaminase >3 times the upper limit of normal. Pregnancy & lactation.
Special Precautions
Liver function test should be performed prior to initiation of treatment, 12 wk following both the initiation of therapy & any evaluation of dose & periodically thereafter. Monitor creatine phosphokinase & transaminase elevations. Increased risk of myopathy in concurrent administration w/ cyclosporine, fibric acid derivative, erytrhomycin, niacin or azole antifungals. Temporary suspension of atorvastatin may be appropriate in serious acute myopathy or patients who have a risk factor for secondary kidney failure to rhabdomyolysis.
Adverse Reactions
Insomnia, headache, nausea, diarrhea, abdominal pain, dyspepsia, constipation, bloating, myalgia, asthenia, hypoglycemia, hyperglycemia, anorexia, peripheral neuropathy, paresthesia, pancreatitis, vomiting, hepatitis, cholestatic jaundice, alopecia, pruritus, skin redness, myopathy, myositis, muscle stiffness, impotence.
Drug Interactions
Mg- & Al- hydroxide containing antacids, antipyrine, cholestipol, digoxin, erythromycin, clarithromycin, azithromycin, terfenadine, OC, amlodipine, PIs.
